Call to Order: The SHS Principal – Ma'am Michelle Sotto and the Senior High Organization ordered
all PUPSHS Students to assemble through MS Teams on October 27, 2022 at 10:00 am.
The meeting started at precisely 10:25 AM, presided over by Aidan Benedict Castillon, the
HUMSSSC Business Manager and Cate Margaret Paspos from the Media Writing Committee.

● Limited face-to-face classes for all levels as students are for pilot testing for the meanwhile,
despite DepEd's Memorandum stating that every school is supposed to be in Full face-to-face
classes by November.
● The University Clinic is located down the East Wing, right in front of the University Chapel.
● Students are not mandated to wear uniforms, but it is highly encouraged by the University for
easy identification.
● Vaccinations are highly encouraged, up until the first booster. If not yet vaccinated yet willing,
contact the medical personnels for assistance.
In compliance with the IATF, DOH policy on alert level system, DepEd order #034 series of 2022, and
University Crisis Management Plan, the following guidelines are hereby adopted and issued for
information and guidance:
2. No face mask, no entry policy shall apply and are required to always wear face masks.
● Despite the announcement of face masks being voluntary indoors, the university
encourages students to wear their face masks inside university premises.
● Face masks can be requested from the clinic in times of emergency.
4. Students and personnel are prohibited from eating together while facing one another. If
there is a lack of space in the classroom or school premises, allow distancing during

meals, and eating shall be in a manner where all individuals face in one direction and do
not talk while their masks are off.
● Students are encouraged to bring their own lunches instead of buying as there is only
one stall as of the moment in the university, personnels also buy at the same stall.
● Students can buy outside of the University premises but only once. They are required to
fill up the HDF again if they've stepped out of the university.
5. Students who have Covid-19 like symptoms shall not report physically, notify their class
adviser, and shift to distance learning if possible. They shall immediately seek
consultation with any medical doctor/private or PUP physician/ any government
accredited medical doctor). Parents/guardians are highly encouraged to monitor and
report their child's health conditions to the school through their class adviser or PUP
medical personnel.
PROCEDURE:
6. The PUP medical personnel may commence contact tracing if necessary and monitor
the health condition of students classified as SUSPECT, PROBABLE, CONFIRMED AND
CLOSE CONTACT of confirmed Covid-19 cases.

7. A report will be submitted by the contact tracer to the task force / Crisis Management
Technical Committee within 24 hours for the CMC (Top Management) recommendation
and appropriate actions.
● Recommendation varies upon the CMC's assessment, in coordination with the City
LGU.
● Lockdowns are per classroom, where the student was exposed of COVID.
8. CMC will coordinate to the LGU / barangay LGU representative for the disposition on a
granular lockdown or any appropriate action.
10. The class adviser will then inform the principal, and the triage officer (Medical Officer)
will initiate contact tracing, proceed with decontamination procedures, or request for
disinfection of the classroom.

VII. Reminders
● It is highly encouraged to read the Revised PUPSHS Policy Guidelines if there are any
more questions regarding protocols, uniforms, grading system, etc.
● Do not forget your Accomplished Health Declaration Form when entering the University
premises. It will be shown/submitted to the guard before entering the premises and on
dismissal.
● Do not forget your personalized health kit.
● Advisers should let their students know their contact information, as they would be the one that
parents will talk to.
● There are approved classroom assignments.
a. Academic Calendar - along with the Revised PUPSHS Policy Guidelines, it is accessible and
downloadable in the webpage.
b. Class Schedule - sets who are not on site during activities will be doing the same activity
according to the professor's instructions.

● Grade 11 students have the whole semester to pass their Form 137. When students
have deficiencies on their record, they cannot enroll for the 2nd semester.
● Grade 12 students are subject to evaluation due to their upcoming graduation by the
second semester. They are urged to pass it immediately as it will be needed for college
applications and their graduation.
e. Uniforms - they are not mandatory, but they are highly encouraged as it is convenient for the
medical personnels and for identification purposes.
