JUSTIFICATION

This is to certify that the intended amount of FORTY THOUSAND FIVE HUNDRED
THIRTY-SIX pesos and 60/100 only (Php 40,536.60) as allocated amount for PSDBM price
quotation and stocks available for the months of April ,May and June MOOE 2021 . it has
been purchased at KSC Enterprises, Poblacion Bayog ,Zamboanga del Sur. Due to non
availability of stocks at PSDBM Ipil Zamboanga del Sur.

1. That the school has send the required forms and pictures of items purchased to
COA emails coadepedngs5steam14@yahoo.com.
2. Until this date no reply received from COA office regarding the procurement
status and receive items purchased in the 2nd.quarter MOOE April to
June,2021.

Therefore, the Bids and Awards Committee (BAC) of Salagmanok Elementary School,
Kumalarang District II, will affix our signature for such justification of sending requirement to
COA office for inspection of items purchased.

JUSTIFICATION
This is to certify that the intended amount of twelve thousand seven hundred three pesos
(Php12,703.00) as allocated in the WFP for the months of July and August 2018 will be used
in the purchase of Pupils and Teachers Supplies for the following reasons:

1. That the budget for school watchman was reallocate for supplies for the
reason that school watchman discontinue his service on June 30, 2018.
2. No approved school LAC Proposal & Session Guides.
3. That these supplies are necessary for pupils’ daily needs and school cards.
4. That the application for internet budget specified in the AIP/WFP for July and
August 2018 is still on process.

Therefore, the Bids and Awards Committee (BAC) of Kumalarang Central Elementary
School - SPED, Kumalarang District, this Division, with the approval of our Principal on August
20, 2018, decided to realign and reallocate the amount stated above for the procurement of
pupils and teachers supplies which are additional items specified in the AIP/WFP, to wit:
