WHEN: 04/04/22 AT APPROX. 3:15 PM

LOCATION: BAYER, CANLUBANG

PRODUCT DELIVERED: ROUGH RICE

WHAT: FAILED TO PUT THE SEAL ON THE DOOR BEFORE LEAVING THE BAYER

NARRATIVE: AS PER OUR DIPATCH UNIT THEY HAD THE REQUEST OF 2 UNITS LAST FRIDAY APRIL 1,
2022 FROM HOLCIM. BUT BECAUSE THERE WAS NO DRIVER AVAILABLE THAT DAY. THEY JUST SERVE THE
FIRST TRUCK SUNDAY EARLY INSTEAD AND THE SECOND TRUCK WAS SERVED JUST YESTERDAY APRIL 4,
2022. PRIOR TO THAT, THE DISPATCH ON DUTY WAS PRESSURED TO SERVE THE SECOND TRUCK AS IT
WAS REQUESTED 3 DAYS AGO AND THE DRIVER OF THAT PARTICULAR TRUCK WAS UNABLE TO REPORT
ON TIME SO THEY DECIDED TO USED A NEWLY HIRED DRIVER ALONG WITH THE HELPER FROM
ANOTHER BUSINESS UNIT. BOTH OPERATIVES ARE NOT FAMILIAR TO THE PROCESS AND NO
ORIENTATION ABOUT PROPER SEALING AFTER LOADING OPERATION. HOWEVER, THERE WAS STILL A
LAPSES ON THE SIDE OF OUR DISPATCH BECAUSE THEY FORGOT TO PROVIDE THE SEAL TO OUR DRIVER
AND HELPER UPON LEAVING OUR YARD.

RECOMMENDATION

 ENSURE SPECIFIC ORIENTATION ARE BEING INPLACE WITH THE DRIVER’S


AND HELPER’S BEFORE ASSIGNING THEM.
 ALWAYS COORDINATE TO THE DRIVER AND HELPER , IF POSSIBLE ASK FOR
PHOTOGRAPH OF MATERIAL TO BE LOAD I.E., BEFORE, DURING AND AFTER
LOADING OPERATION
 AS MUSH AS POSSIBLE NOT TO USE NEWLY HIRED DRIVER WITHOUT
APPRENTICESHIP/APRRENTICE TOUR TO THE SPECIFIC SITE DELIVERY.
 PRIOR THE DEPARTURE, INSTRIUCTION FROM THE DISPATCH MUST BE
PROVIDED

and helper upon delivery of the product. We understand that handling
of pressure is part of our daily task and we can’t used this as an excuses
