Fig2-1A: Ta thở air (inspiration) vào khí quản (trachea), phế quản (bronchi,
bronchioles) tới phế nang alveolus rồi O2 qua màng phế nang alveolar membrane
vào blood vessel. Máu mang O2 đi khắp cơ thể rồi chở thán khí CO2 về phổi qua
alveolar membrane vào phế nang alveolus rồi thở ra (expiration) qua phế quản
(bronchi, bronchioles), khí quản (trachia) đi ra ngoài (lúc bình thường normal). Việc
đó gọi là air diffusion thru alveolar membrane sẽ bị trở ngại trong trường hợp
membrane destruction (emphysema) hay membrane thickening (cystic fibrosis).
Máu bị thiếu O2 nên phải thở mạnh khi làm việc nặng exertional dyspnea như trong
CAD (coronary artery disease).
Vậy SOB (exertional dyspnea) là do tim hay phổi là thủ phạm.
Fig.2-1 B: Nếu alveoli bị đầy tràn những thứ khác, làm giảm lượng O2 trong
máu:
* Pus trong pneumonia.
* Fluid trong edema:
- cardiogenic pulmonary edema: suy tim (heart failure)--> tim bóp yếu-->
máu ứ ngược lên phổi--> nước toát ra alveoli, chiếm chỗ của O2.
- non cardiogenic pulmonary edema: trong acute respiratory distress
syndrome ARDS (septic shock, trauma,..)--> inflammation gây ra leaking
pulmonary capillaries (mao quản bị hở)--> nước toát ra alveoli-->
pulmonary edema
- blood: pulmonary hemorrhage trong lung cancer hay Goodpasture's
syndrome (hemoptysis, hematuria)
Fig.2-1C: túi nang alveolar sac không mở ra đủ do óc không bảo alveolus mở
ra (central sleep apnea) hay do restrictive diseases (stiff chest wall, respiratory
muscle weakness, stiff lungs)--> airway cannot open all the way, difficult to keep
open and collapse quickly--> restricted inspiration
Phân biệt Central sleep apnea với Obstructive sleep apnea:
- Central sleep apnea: sau khi thở hết ra rồi nín thở lại, chỉ một lúc sau ta phải bật
thở (gasp for air) vì tình trạng hypoxia (thiếu dưỡng khí) hay hypercarbia (nhiều thán
khí quá) đòi hỏi ta phải thở--> respiratory center trong brain stem "bảo phổi hãy thở
đi!" Nếu óc (central) khi ngủ không nhậy cảm nhận biết thì túi nang (alveolar sac)
không mở ra để khí vào--> central sleep apnea (tắc thở khi ngủ).
- Obstructive sleep apnea: do upper respiratory airways bị bít lại khi ngủ.
Obstructive sleep apnea occurs when the muscles that support the soft tissues
in your throat, such as your tongue and soft palate, temporarily relax. When these
muscles relax, your airway is narrowed or closed, and breathing is momentarily cut
off.
Cả 2 trường hợp vì ban đêm hay thức giấc do apnea (tắc thở) nên ban ngày mắc
chứng buồn ngủ (rất nguy hiểm trong khi lái xe).
Fig.2-1D: Thở ra khó difficult expiration vì:
- obstruction of the airway: foreign body mắc ở khí quản, tumor, chronic
mucus plugging (chronic bronchitis, cystic fibrosis-->mucus bít phế quản và
pancreas), asthma (constrictive airway due to cold, emotion, allergenes, exercise),
emphysema (cấu trúc phổi bị hư hại, mất tính đàn hồi, không thở hắt thán khí ra
được--> phổi tắc hơi căng khí--> ngực to ra)
*Cystic fibrosis is a hereditary disease that affects the lungs and digestive
system. The body produces thick and sticky mucus that can clog the lungs and
obstruct the pancreas. Cystic fibrosis (CF) can be life-threatening, and people with
the condition tend to have a shorter-than-normal life span.
*Asthma is a condition in which your airways narrow and swell and may
produce extra mucus. This can make breathing difficult and trigger coughing, a
whistling sound (wheezing) when you breathe out and shortness of breath.For
some people, asthma is a minor nuisance. For others, it can be a major problem
that interferes with daily activities and may lead to a life-threatening asthma attack.
*Emphysema is a lung condition that causes shortness of breath. In people
with emphysema, the air sacs in the lungs (alveoli) are damaged. Over time, the
inner walls of the air sacs weaken and rupture — creating larger air spaces instead
of many small ones. This reduces the surface area of the lungs and, in turn, the
amount of oxygen that reaches your bloodstream. When you exhale, the damaged
alveoli don't work properly and old air becomes trapped, leaving no room for fresh,
oxygen-rich air to enter. Most people with emphysema also have chronic
bronchitis. Chronic bronchitis is inflammation of the tubes that carry air to your
lungs (bronchial tubes), which leads to a persistent cough. Emphysema and chronic
bronchitis are two conditions that make up chronic obstructive pulmonary disease
(COPD). Smoking is the leading cause of COPD. Treatment may slow the
progression of COPD, but it can't reverse the damage.
Fig.2-1E: - Blood: Nếu do chính blood không mang đủ O2 (anemia) -->phải thở
mạnh--> SOB & tachycardia. Vậy [SOB & tachycardia] không phải chỉ do Tim
hay Phổi mà do cả Máu nữa.
- Blood Vessels:
@ Left heart (failure, valvular diseases): tim không bóp hết máu forward,
bị ứ lên phổi.
@ VSD (ventral septal defect: vách tâm thất có lỗ hở) máu chạy từ Left
ventricle sang right ventricule.
@ Interstitial lung diseases is the name for a large group of diseases that
inflame or scar the lungs.The scarring is called pulmonary fibrosis.
Breathing in dust or other particles in the air is responsible for some types
of interstitial lung diseases. Specific types include
-Black lung disease among coal miners, from inhaling coal dust
-Farmer's lung, from inhaling farm dust
-Asbestosis, from inhaling asbestos fibers
-Siderosis, from inhaling iron from mines or welding fumes
-Silicosis, from inhaling silica dust
Treatment depends on the type of exposure and the stage of the disease. It may
involve medicines, oxygen therapy, or a lung transplant in severe cases.
@ In one form of pulmonary hypertension, called pulmonary
arterial hypertension (PAH), blood vessels in your lungs are narrowed
(arteriosclerosis), blocked (pulmonary embolism) or destroyed (vasculitis).

còn lại trong phổi sau khi đã thở ra hết sức vì áp xuất trong lồng ngực intrathoracic
pressure vượt quá pressure in the airways, làm cho airways xẹp xuống)
@ Restrictive Lung Disease: hít vào giới hạn restricted inspiration--> total
lung capacity giảm--> FEV1 và FVC đều proportionally decreased--> FEV1/FVC
không thay đổi hay tăng (vì sức thở ra còn mạnh nên FEV1 giảm ít hơn FVC) trong
các bệnh restrictive lung diseases so với người bình thường.
- Emphysema (vì alveolar membrane bị tan nát--> alveolar surface giảm) nhưng
không giảm trong các obstructive lung diseases khác.
- Pulmonary Fibrosis (thick alveolar membrane-->giảm rate of uptake --> giảm
DLCO). Decreased DLCO is a hallmark of pulmonary fibrosis.
- Anemia (decreased rate of uptake across the alveolar-capillary membrane due to
the lack of hemoglobine)

Description
Cystic fibrosis affects the cells that produce mucus, sweat, and digestive
juices. It causes these fluids to become thick and sticky. They then plug up tubes,
ducts, and passageways.
Symptoms vary and can include cough, repeated lung infections, inability to
gain weight, and fatty stools.
Treatments may ease symptoms and reduce complications. Newborn
screening helps with early diagnosis.
Cystic Fibrosis là bệnh di truyền--> rối loạn chloride và water flow-->
secretion of viscous mucus (bít phổi--> khó thở, infection, bít pancreas--> thiếu
enzyme tiêu hóa chất mỡ gây floating fatty stools (phân nổi lềnh bềnh và rất thối)
đôi khi gây chứng Diabetes Mellitus vì bít insulin secretion, bít nhiều cơ quan khác
như gan mật --> sỏi mật, bít ruột--> malabsorption--> trẻ em chậm lớn, mồ hôi mặn
nhiều muối (chloride tăng).
Chẩn đoán: chloride tăng trong mồ hôi. Chữa trị: DNAse enzyme để break
down mucus, pancreatic enzyme, dietary

Lung Cancer
- Pathology: có 4 loại ung thư phổi: small cell lung cancer and non-small
cell lung cancer (large cell carcinoma, squamous cell carcinoma, adenocarcinoma).
Small cell lung cancer là nặng nhất, khi chẩn đoán được thì đã có metastasis nên
thường phải dùng chemotherapy và thường hay kèm theo chứng paraneoplastic
syndromes như SIADH (--> ectopic ACTH--> cortisol--> Cushing syndrome).
- Symptoms & signs: pulmonary symptoms (cough, dyspnea, hemoptysis),
constitutional symptoms (fever, weight loss, fatigue, night sweat), symptoms related
to compression of nearby structures [như đè vào khí quản--> dysphagia, đè vào
recurrent laryngeal nerve--> giọng khàn hoarseness, đè vào superior cervical
ganglion--> Horner's syndrome (ptosis, myosis, anhidrosis), đè vào SVC--
> superior vena cava syndrome: facial/arm swelling, headache orthopnea (phải
đứng lên mới bớt nhức đầu và khó thở), jugular venous distension].
-Treatment: surgery, chemotherapy, radiation tùy theo size, local extent,
lymphe node involvement và distant metastasis.
- Pneumothorax: alveoli bị vỡ--> hơi thoát ra pleural space (do trauma dao
đâm, iatrogenic manipulations như mechanical ventilation/thoracentesis hay tự
nhiên spontaneous pneumothorax)--> asymptomatic (nếu nhỏ), tức thở và đau lắm
(nếu lớn như hình sau):
Đó là tension pneumothorax, tức thở và đau lắm, phải đặt chest tube vào cho
hơi thoát để phổi dãn ra mới thở được.
- Hemothorax: máu tràn vào pleural space do trauma chấn thương
hay ruptured aortic aneurysm.
- Chylothorax: lymph tràn vào pleural space do trauma to or obstruction of
the thoracic duct.
Lời cuối cùng là gặp bệnh nhân nào khó thở, ngoài việc lấy vital signs
(temperature, pulse, BP, nhịp thở RR) phải đo
O2 saturation % bằng pulse oximeter nếu dưới 90% là phải cho hít O2 ngay.
