    Based on a survey of 324 respondents, the following ordinal logistic regression model was generated: 1) Perceptions of patient safety culture among health workers were generally good, with 88% responding positively. 2) Cooperation among work teams, communication, cooperation between units, frequency of incident reporting, and management support also received largely positive responses (between 78-88%). 3) The model was able to distinguish between response categories with good fit and predictive ability.
