Construction and Erection of National Grid Corporation of the Philippines (NGCP)
Transmission Lines affected by the Department of Transportation (DOTr) South
Commuter Railway Extension Project

i. The hazards outlined in this activity applies for the Construction and Erection of
Transmission Lines, and workers who are experienced in the use of Excavating and
Concreting Equipment, hand and power tools.
ii. Workers involved in excavation and concreting shall use related procedure as preliminary
step toward concrete pouring. The same workers shall be involved in working at height,
deep manual excavation as necessary to construct concrete foundation and erect
Transmission Lines.
iii. Workers conducting this operation may be employed by Maxipro Development
Corporation (MDC), Project-Based Hired Independent Local Workers or MDC Admin-
Team.
 This activity sheet does not provide an in-depth analysis of OSHA Standards and
Regulations. It is intended as a guide and quick reference for NGCP, Contractor, Sub-
Contractor workers. The matrix captures major activities involved in the Construction
and Erection of Transmission Lines. NGCP and the Contractor must evaluate the
specific hazards associated with the operation at the site where the work is being
performed. MDC, the Contractor, prepared and shall submit a matrix of Job Hazard
Analysis Plan and Job Safety Analysis as reference to Annex A, considering actual site
condition.

I. Purpose

To describe the requirements and methods that shall be employed and equipment
that shall be deployed in the Construction and Erection of Transmission Lines.

II. Scope

This methodology is applicable only to Maxipro Development Corporation or its Sub-


Contractors performing the works in the Construction and Erection of Transmission
Lines.

III. Responsibility

The Project Supervisor shall be responsible that this methodology is understood,


adhered to, and employed by all personnel involved in all activities.

IV. Safety Considerations

This includes all safety practices that must be implemented, safety signages, safety
barriers, mandatory use of PPE’s, and COVID-19 Protocols. The following shall be
implemented and executed on site:

A. Supervisors/Foremen shall inspect the working area for any additional safety risk
before holding the Toolbox Talk/Risk Analysis meeting and shall be discussed in
the meeting.

B. Supervisors/Foremen shall hold Toolbox Talk Meeting with the construction team
in the morning prior to the commencement of any work, all participants shall sign
the Attendance Sheet and understanding thereof.

C. If after the evaluation by a Geotechnical Engineer/Client Project Engineer, the


soil appears to be unstable in the excavation walls, shoring shall be done.
Alternatively, the upper half of the excavation shall be excavated at an angle of
repose of 60° to avoid the sides to collapse.

D. Access ladder to the excavation shall be utilized whenever necessary. Access


ladders shall be inspected before use.

E. Reflective Barriers shall be installed one (1) meter away from the mouth of the
excavation, circumferencing the excavation to avoid any untoward incident.

F. The construction site shall be fenced using G.I. Pipes and G.I. Sheets or
Bluesheets one (1) m away from the existing canal to avoid impedance to traffic
and to avoid traffic bottleneck in the construction site. Please refer to Figure 1.

G. Applicable Safety Signages shall be installed in the construction site.

H. Supervisor shall inspect the construction site prior to departure to ensure that the
area is safe.

I. Personal Protective Equipment (PPE) shall be properly used at all times during
execution of construction activities.

J. COVID-19 Safety Protocols shall be strictly implemented before, during and after
works, based on DTI and DOLE Interim Guidelines Prevention & Control of
COVID-19 Community Transmission, as follows:

● Shall conduct information dissemination about COVID-19 Pandemic


● Shall conduct Routine Environmental Cleaning
● Shall require mandatory use of facemask at all times
● Shall provide workers with disinfectants such as hand sanitizers and the likes
● Physical distancing shall be observed at all times
● Suspected COVID-infected worker shall be isolated and disinfection activities
shall be employed in the construction site to avoid transmission
● Body Temperatures shall be checked prior to the entry in the construction site.
● Disinfection of vehicle/equipment in the hub operational area shall be done

V. Access Works

This shall include negotiations with landowners for the access to reach each pole
position. Maximum use of existing access and existing roads (public or private) shall
be observed. Road width shall be determined by need, such as equipment size, and
shall be no wider than necessary. Particularly, the following method statements shall
be implemented:

A. Agreed access route shall be cleared of all slumps, tree branches and/or other
rubbish. Suitable culvert pipes shall be placed at the crossing of the channels or
natural waterways.

B. Access specifications such as thickness and width of filling materials shall be


determined considering the construction equipment that will traverse the access to a
particular tower site and as agreed by the landowner. The access road shall be
properly maintained during the course of the construction.

C. For the sites located close to existing roads, the existing road shall be utilized as
access to the construction site. In some cases, repairing of the existing road shall be
required such as partial filling of existing potholes and/or installation of culvert pipes
crossing over the drainage or other measures as required. Consent from the
local/private owners shall be secured prior to use. Existing canals for drainage or
irrigation shall be protected by providing steel plate covers.

VI. Foundation Works

These include, but not limited to, construction of foundations, site clearing, survey
lay-out, excavation works, reinforcement bar setting/installation, formworks, anchor
bolt setting, scaffolding works, installation of groundings, concreting, backfilling
including tampering and site leveling and grading works. Particularly, the following
methodologies for each activity shall be implemented:

A. Site Preparation

Prior to the commencement of civil works route/alignment shall be established. As


per Technical Specification of the project, Right of Way (ROW) clearing shall be
done. If the clearing involves cutting down of trees and vegetation, permits from the
Department of Environment and Natural Resources (DENR) shall be secured. Trees
shall be cut not higher that 15cm above ground level and all stumps that may cause
obstruction during excavation activities shall be removed using excavators or by
other means. All trees that have been cut down shall be stockpiled in an approved
area.

B. Excavation Layout

The Surveyor shall check all reference points available in order to determine the
actual steel pole location. As reference during excavation, anchor bolts settings, and
concreting works, batter boards shall be erected.

C. Excavation by Machine

1. A reference point elevated from the ground surface for measuring the depth
of excavation shall be established. For every excavation pit, two references
points of the same elevations shall be established.
2. Approved dumping areas for excavated materials shall be identified. These
excavated materials shall later be utilized for backfilling if found suitable.
3. Sufficient number of water pumping equipment shall be provided to ensure
dry execution of excavation works. If required, temporary drainages around
excavation pits shall be constructed.
4. In case the results of Standard Penetration Test reveal that the soil is soft
and non-cohesive, excavation shall be executed using open cut excavation
method. In this case, the sides of excavation pit shall be sloped to an angle
of approximately 60°.
5. Excavation shall be square, plumb to depths. Usually, the leveling of bottom
of excavation as well as trimming shall be done by handwork.

D. Installation of Rebars

The length, grade and size of steel to be installed shall be according to contract
drawings and specifications. Pre-fabricated reinforcements shall be lowered centrally
into the pits and shall be casted in place above the lean concrete, using G.I. tie
wires.

E. Concreting

Ready Mic Concrete shall be used since the project location is accessible. The
concrete mix design and strength shall be as specified as per plan. Three (3)
Cylinder samples shall be collected for testing, it shall be collected at 30% pouring,
60% pouring, and 90% pouring respectively. It shall be transported up to the
accessible area by transit mixers with enough interval to ensure efficient flow of
activities. Prior to concrete pouring, checking and cleaning of pits shall be done.
Mechanical vibrators shall be used during the pouring of concrete. Vibrations shall be
done in regular intervals to eliminate possible voids. Chutes and/or excavator
buckets shall do the placing of the concrete mixture into the formworks depending
on the actual site condition.

F. Removal of Formwork

Formworks shall be removed as soon as practicable in order to avoid curing and to


employ repair of surface, but in no case shall they be removed without approval.
Care shall be taken as not to damage the concrete and cause any shocks to the
structure especially anchor bolts. When it is intended that formwork is to be reused,
it shall be cleaned properly for reuse. If voids are found in the surface of the
concrete, it shall be plastered after removal of formworks.

G. Backfilling Works

All formworks and materials used shall be removed prior to start of backfilling works.
Excavation pits shall be cleared of all construction debris. Mud, rubbish, timber and
other debris shall be removed from the soil that shall be used as backfilling material.
Backfilling shall proceed in layers not exceeding 300mm in thickness. In order to
achieve a minimum of 80% compaction, each layer of shall be rammed.

H. Reinstatement of Steel Pole Site

After completion of the foundation works, the site shall be properly cleared of all
construction debris (such as concrete remains, disposed formwork, supports, nails,
wire etc.) The site shall be properly leveled and graded as far as practicable to its
original condition. Areas surrounding the steel pole site and was damaged due to the
construction of foundations including agricultural areas that were utilized as
temporary accesses shall be reinstated as much as possible to its original condition.

Roads used for delivery of construction materials shall be ensured passable in order
for other construction works to proceed without interruption.

I. Traffic Management During Foundation Works

1. Traffic Safety Signages shall be placed 15meters away from access to the site to
ensure passing traffic to slow down due to the on-going construction works.
2. Traffic Safety Signages shall be placed 15meters away from access to the site
during concrete pouring to warn the passing traffic of the presence of Heavy
Equipment in the area.
3. Flagmen shall direct traffic especially during concrete pouring to avoid any
untoward traffic accidents. Flagmen shall be provided with 2-way m radio
transceiver for communication purposes with other flagmen and the site
supervisor. Please refer to Figure 2 for the proposed traffic management plan.

VII. Pole Erection Works

These include hauling of pole parts from the main stockyard, sorting, counting, &
checking of delivered pole parts, erection of each steel pole using crane, pole
erection inspection and correction works.

VIII. Installation of Additional Grounding

These include installation of grounding rods, connecting wires and clamps as well as
measurement of grounding resistance.

IX. Personnel

It shall be ensured that only well-trained workers and skilled individuals such as
flagmen and spotters supervised by a competent Site Supervisor and Safety Engineer
shall perform the Steel Tower Pole Construction.

X. Permit to Work

All relevant permits per activity shall be secured prior to the commencement of any
construction activity. The acquiring of permits shall be channeled through properly.

XI. Equipment

All necessary equipment to do the works shall be employed at the site ensuring that
they are only operated by authorized and experienced operators with NCII
Certifications together with spotters. All Personal Protective Equipment shall be
provided for all personnel as follows:
 Safety Helmet
 Safety shoes/Boots
 Hi-visibility vest/clothing
 Gloves
 Eye Protection

XII. Access and Egress for the Work Location

The outer perimeter of the excavated shall be allocated for access. Where required,
it shall be provided by barriers. Spotters and Flagmen shall be provided with flags or
any visible communication device with the Operators.

XIII. Communication

Communication during the execution of work between the Supervisor and personnel
shall be verbal. In some cases, written communication shall be prepared to ensure
proper documentation of critical activities. The foremen shall update the Supervisor
of progress of works on a daily basis. The Site Supervisor and Safety Engineer shall
be available for communication via mobile phones.

XIV. Weather Conditions

The prevailing weather condition at the site shall grossly affect the foundation works.
The working conditions at site shall be monitored on a regular basis by the Site
Supervisor. The decision to continue or stop the operations shall be made by the Site
Supervisor.

XV. Emergency Procedure

All untoward incidences shall be reported immediately to Site Supervisor. Action shall
be taken as per company Health and Safety Policy. In such cases, the following are
personnel and equipment and the location of the nearest health facility are available:

 First Aid Box Location: Site Area
 Location of Nearest Hospital: The Medical City Clark

XVI. Other Information

All workers shall be responsible for maintaining their Personal Protective Equipment.
Manual handling of activities shall be kept minimum and mechanical handling shall at
all times be available. All lorries shall use a Banksman/Flagman to enter and leave
the site, making sure all loads are safe.

XVII. Technical Specification Coverage

All the works of the project shall be carried out in accordance with the Technical
Specification of Contract Documents by the Client hereinafter called “Specification:
and relevant standards, as well as in the compliance with requirements of Health,
Safety and Environment.
