AFFIDAVIT OF LOSS

REPUBLIC OF THE PHILIPPINES)

SANTIAGO CITY ) S.S.

THAT, I, BENIGNO M. ALIPIO, of legal age, Filipino, married to Evangeline J.

Alipio and a resident of P-2, Buena Vista, Santiago City, after having been duly sworn
to in accordance with law hereby depose and states:

1. That I am the owner of a certain parcel of land located in Purok 2,

Valentin St., Buenavista, Santiago City under Transfer Certificate of Title
No. TSC-3741 and more particularly described as follows:

“A PARCEL OF LAND ( Lot No. 461-A-2-A-2-A of the subd. Plan Psd-

(af)-02-021482 being a portion of Lot 461-A-2-A-2, Psd -02-055507,
situated in the Barrio of Buenavista, City of Santiago, Island of Luzon.
Bounded on the NE., along line 1-2 by Valentin Street; on the SE., along
line 2-3 by Lot 461-A-2-B, Psd-2-02-022060; on the SW., along line 3-4 by
Lot 461-B, (LRC) Psd-191091; on the NW., along line 4-5 by Lot 461-A-2-
A-2-B of the subd. Plan and along lines 5-6-1 by Lot 461-A-2-A-1 Psd 2-02-
055507. Containing an area of THREE HUNDRED SEVENTY NINE (379)
square meters, more or less.”

2. That in October of 2016, a strong typhoon named “LAWIN”, given its

massive strength, “LAWIN” brought strong winds with very heavy rain
that caused massive flood in the whole region;

3. That during the surge of typhoon “LAWIN”, water entered our house
and destroyed some of our cabinets and appliances;

4. That in November of 2016, during the aftermath of typhoon “LAWIN”,

while cleaning our house I happened to check on our records and all the
 papers in our file were torn and I was not able to locate the owner’s
original copy of the above-described property because of its condition;

5. That I executed so much effort to look for the said owner’s original
copy, nevertheless, it was all in vain, hence I now consider the
same as lost and beyond recovery.

6. That I am executing this affidavit to attest the truth in the foregoing and
in order to request the reissuance of another owner’s original copy of
the above-described property.
