“A RESOLUTION ALLOCATING THE AMOUNT OF TWELVE THOUSAND PESOS (P 12,000.00) FROM

THE 20% BARANGAY DEVELOPMENT FUND FOR THE OPERATION AND MAINTENANCE OF
SETTING-UP OF TEMPORARY ISOLATION FACILITY WITH PROVISION OF AMENITIES, PPEs and
CLEANING MATERIALS KALAHI-CIDSS NCDDP DROM/P SUB-PROJECT AT BARANGAY
SACRAMENTO VALLEY, MUNICIPALITY OF SAN FERNANDO, BUKIDNON.”
WHEREAS, the Operation and Maintenance Action Plan is required for the sustainability of the completed
sub-project.

WHEREAS, the estimated total annual cost of the Operation and Maintenance of the Setting-up of
Temporary Isolation Facility with Provision of Amenities, PPEs and Cleaning Materials Sub-Project in
accordance with the Operation and Maintenance Action Plan proposed by the O&M Group is Twelve
Thousand Pesos (12,000.00).

WHEREAS, the Operation and Maintenance Plan was duly approved by the BSPMC and Barangay
Disaster Risk Reduction Management Council of Barangay Sacramento Valley, Municipality of San
Fernando, Bukidnon.

NOW THEREFORE, on motion of Hon. Cresente C. Ortiz, Barangay Kagawad and unanimously seconded
by all members present, it was

RESOLVED, as it is hereby resolved to allocate the amount of Twelve Thousand Pesos (12,000.00) from
the 20% Barangay Development Fund for the Operation and Maintenance of the Setting-up of Temporary
Isolation Facility with Provision of Amenities, PPEs and Cleaning Materials KALAHI-CIDSS NCDDP
DROM/P Sub-Project at Barangay Sacramento Valley, Municipality of San Fernando, Bukidnon

RESOLVED FURTHER, that this resolution be copy furnished to KALAHI-CIDSS NCDDP ACT office,
DSWD Region X Field Office and to Local Government Unit of San Fernando, Bukidnon.
