“RESOLUTION AUTHORIZING THE MUNICIPAL MAYOR, HON. SOLOMON T. RUFILA FOR AND
IN BEHALF OF THE LOCAL GOVERNMENT UNIT OF SAN FRANCISCO, AGUSAN DEL SUR TO
ENTER INTO A MEMORANDUM OF AGREEMENT (MOA) WITH THE SAN FRANCISCO
DOCTOR’S HOSPITAL, INC., REPRESENTED BY IT’S HOSPITAL ADMINISTRATOR, ATTY. NEIL B.
JAO, MA, MBA, FPCHA FOR THE ANNUAL MEDICAL AND DIAGNOSTIC SERVICES PACKAGE
FOR OFFICIALS AND EMPLOYEES OF THIS MUNICIPALITY PURSUANT TO ORDINANCE NO. 70,
SERIES OF 2017 DUBBED AS ‘ AN ORDINANCE INSTITUTIONALIZING THE EMPLOYEES’
HEALTH CARE AND WELLNES PROGRAMS OF THE MUNICIPALITY OF SAN
FRANCISCO,AGUSAN DEL SUR AND APPROPRIATION”

WHEREAS, the San Francisco Doctor’s Hospital, Inc. herein after referred to as
“SECOND PARTY”, is a Level II Hospital duky organized and existing under and by
virtue of the laws of the Philippines, with business address at National Highway,
Barangay Hubang, San Francisco, Agusan del Sur;

WHEREAS, the “SECOND PARTY” agrees to provide medical, dental, optical and
diagnostic services to the bonafide beneficiaries of the “ FIRST PARTY” under the package
articualted in the MOA;

WHEREAS, UPON THE EFFECTIVITY OF THE MOA, the “FIRST PARTY” shall submit to the
SECOND PARTY a list of benefeciaries under the Agreement, properly indicating therein the
employment status of each beneficiary;

WHEREAS, further, the “ FIRST PARTY” shall pay its liability to the SECOND PARTY for
services rendered within fifteen days (15) receipt of the billing statement.

NOW THEREFORE, on motion of SBM EDWINO A. DANDAN and duly seconded by SBM
CARLITO P. TANDOG, be it ---

RESOLVED, as it is hereby resolved, to authorize the Municipal Myor, Hon. Solomon T.

Rufila for and in behalf of the Local Government Unit of San Francisco,Agusan del Sur to
enter into a Memorandum of Agreement ( MOA) with the San Francisco Doctor’s Hospital,
Inc., represented by its Hospital Administrator, Atty. Neil B. Jao , MA, MBA, FPCHA for the
Annual Medical and Diagnostics services package for official and employees of this
municipality pursuant to Ordinance No.70, series of 2017 dubbed as “An Ordinance
Institutionalizing the Employees’ Health Care and Wellness Programs of the Municipality of
San Francisco, Agusan del Sur and providing appropriation hereof”.

APPROVED BY THE SANGGUNIANG BAYAN OF SAN FRANCISCO, AGUSAN DEL SUR, in its
regular session held on August 10, 2020.
