CO = CONSEQUENCE OF OCCURRENCE - RISK LEVEL = HIGH (H) - MEDIUM (M) - LOW (L)
Note that should an H – High PO and an H – High CO be recorded together for a risk
exposure, the risk is unacceptable and further control measures are required.

 Slips, Trips and Falls – the most 
common reason for accidents is poor
housekeeping standards, such as build up
of waste debris, oil or grease on floors and
poor cable management.
The highest standard of housekeeping shall be
maintained and shall include:
- Debris shall be removed on a daily
basis
- All spillage’s such as oil or grease
etc. shall cleaned up immediately
- Cables shall be managed in such a
way that where possible they shall be
off the floor areas and especially
walks ways.
- Supervisors shall monitor the
standards of housekeeping to ensure
they are acceptable and implement
routine clean-ups where necessary.
