Brief Description : During construction of a scaffold platform two single scaffold clips from the same new batch
failed while being tightened up. It appears from initial inspection that the materials these are
made from are too soft / malleable. One rivet holding the bolt into the main part of the clip
has sheared and one has been pulled out. Both have deformed the hole in the main body of
the clip.

The clips were manufactured for CAPE Offshore Services in September 2000 and the Batch
number is G-784.

Analysis
Actions Taken : As this is a new batch of scaffold clips recently arrived on the platform we can easily identify
where they have been used. All scaffolds where these clips have been used have been
quarantined and Scaffold Tag removed. All clips will be examined and any from this batch
will be replaced.
