1. SCOPE

This method of statement describes the procedure for


Dismantle & Installation of instruments at CRISP Package 1 Process at 51 Jurong Island
Highway existing HGIW Plant.
Dismantle & Installation of field Instruments and its accessories, Cable support, Cables,
Glanding & Termination, junction box and tubing according to approved drawings.
Inspection & housekeeping.

2. PURPOSE

This work method of statement covers all the below given works in CRISP Package 1 process
Project scope of works. This document defines the method of statements and requirements
to, dismantle & installation of field Instruments and its accessories, Cable support, Cables,
Glanding Termination, junction box and tubing according to approved drawings.

Do the necessary inspection upon completion of the above work.

4. ROLES & RESPONSIBILITES

4.1. Project Manager


4.1.1. Familiarize himself with the Company’s environmental, health and safety
management system and ensure its effective application.
4.1.2. Review all working methods and processes, both new and existing, to see that all
environmental, health and safety are met.
4.1.3. Ensuring necessary resources are arranged according to project schedule.
4.1.4. Take prompt corrective actions to remove any unsafe or unhealthy work conditions
and to maintain effectively such correction made.
4.1.5. Establish work plan and staffing for each phase of project and arranges for
recruitment or assignment of project personnel. Confer with project staff to outline
work plan and to assign duties, responsibilities, and scope of authority.
4.1.6. Direct and coordinates activities of project personnel to ensure project progress on
schedule and within prescribed budget. Reviews of status reports prepared by
project personnel and modified schedules or plans are required.
4.1.7. Ensuring that all necessary preparation and documents are approved prior to
commencement of the work.
4.1.8. Prepares project report for management, client and others.
4.1.9. Confers with project personnel to provide technical advice and to resolve problems.
4.1.10. May coordinate project activities with activates of government regulatory other
government agencies.
4.1.11. Ensuring that safe work practice and risk assessment are established and
implemented.

4.2. WSH MANAGER / OFFICER


4.2.1. Ensure that all rules and regulations as stipulated in the WSHA (Construction)
Regulations are met.
4.2.2. Plans, implements, and coordinates program to reduce or eliminate, occupational
injuries, illnesses deaths and financial losses: Identifies and appraises condition
which could produce accident and financial losses and evaluates potential of given
system, operation, or process.
4.2.3. Conducts or directs research studies to identify hazards and evaluate loss
producing potential of given system, operation, or process.
4.2.4. Develops accident prevention, loss control system and security programs from
incorporation into operational policies organization.
4.2.5. Maintains liaison with outside organizations such as fire departments, mutual aid
societies and rescue teams to assure informant exchange and mutual assistance.
Develops methods to evaluate safety program and conducts or directs evaluations.
4.2.6. Ensure that a project risk assessment is complete as is required by Singapore
legislation and is reviewed periodically.
4.2.7. Ensuring a training needs analysis is undertaken and the necessary training.

4.3. PROJECT ENGINEER


4.3.1. Familiarize himself with the Company’s environmental, health and safety
management system.
4.3.2. Review any new or changes in processes, equipment, and materials to ensure that
safety, health, and environment are not disregarded.
4.3.3. Ensure work is being carried out in accordance with the safe work practice and
method of statement.
4.3.4. Establish and maintain a programmed for the identification and assessment of
hazards.

4.4. WSH COORDINATOR

4.4.1. Undergone and possess valid Certificate on WSH Co-coordinator Course and
OPSOC Course
4.4.2. Liaise with the Client representative on matters relating to health, safety security
and environmental aspects of the project.
4.4.3. Inspect and rectify any unsafe place of work and unsafe practices.
4.4.4. Assist the Safety Manager, Workplace Safety and Health Officer and Environmental
Control Officer in all safety, health & environment matters.
4.4.5. Carry out site safety, health and environment programs and procedures.
4.4.6. Coordinates and assures compliance with the Risk Assessment Program.
4.4.7. Ensure that the issuance of personal protective equipment is properly recorded and
documented.
4.4.8. Coordinates and assures compliance with the Risk Assessment Program
4.4.9. To ensure all personnel working at site attended SembCorp in-house SIC.

4.5. SUPERVISOR / FOREMAN

4.5.1. Conduct daily Pre-task talk before commencement of work.


4.5.2. Ensure that workers know safety, health and environmental rules and regulations,
follow safe job procedures and be aware of all major hazards associated with their
work and work areas.
4.5.3. Responsible for the safety, health & welfare of all workers either assigned or not
assigned to him but may be working temporarily in his area.
4.5.4. Maintain satisfactory standards of housekeeping in his assigned area.
4.5.5. Investigate all incidents to workers and or equipment in his area and assist
Workplace Safety and Health Officer to put up accurate incident report on all such
investigation.
4.5.6. Ensure that all workers working in this area use the appropriate personal protective
equipment.
4.5.7. Ensure all PTW is approved, and all safety precautions are implemented as
indicated in the PTW.
4.5.8. To make sure the area where the work is to be carried out is barricaded out and
ensure safety.

4.6. GENERAL WORKERS

4.6.1. Accepting individual responsibility for their own safety behavior as well as sharing
responsibility with their fellow workers and line supervision to look out for their own
as well as their co-workers.
4.6.2. Assist the skilled personnel / workers during the execution of assign task by the
Foreman and ensure that he understand and complied all the safety measures and
precautions.
4.6.3. Attend daily toolbox/ pre-task meetings.
4.6.4. Inspect all tools before use and daily.
4.6.5. To report potential hazard to supervisor.

6. WORK ACTIVITY

6.1. Preparatory Works


• Ensure that all the site personnel must be trained (OPSOC, SIC & other applicable
courses)
• All the necessary documents (MSRA & necessary Drawings must be prepared &
approved by SUT prior to work starts).
• Conduct the site survey to mark on the plan and seek approval for identified/planned
location of job from SUT.
• All the necessary Permit to Work (PTW) & authorization forms to be prepared &
submitted for approval one day in advance as per SUT norms. (Repeat this until the
work is completed).
• The additional PPE’s must be prepared in advance, if required as per the specific PTW.
Example (Rubber gloves).
• Secure the necessary approved PTW / authorization forms before the work starts every
day.
• Ensure that hazards and or unsafe conditions are addressed and discussed to avoid
hazards and or unsafe conditions prior to work starts.
• List out the tools, materials and equipment required for the specific job and mobilize it to
the job site and check they are in good working condition with valid inspection report if
required.
• Barricade the working area and arrange the materials inside the barricade prior to start
work.
• 4 in 1 gas monitor must be prepared and perform bump test prior to each day use.
• Ensure to close the permit upon completion of work as per SUT procedure.

6.2. Dismantle & Installation of field instrument, Panels, tubing, junction box, support &
Cables
• Report to site supervisor attending daily toolbox meeting.
• Obtain all necessary permits.
• Ensure PPE, to be worn.
• All hand tools to be used at site must be inspected and approved prior to use, check for
monthly color code.
• Ensure correct location where field instrument, Panel, JB and tubing to be dismantled &
installed properly identified according to updated drawing.
• Verify the tag & location as per P & ID.

• Check voltage using multimeter and isolate the fuses on panel side as per drawings.
• Ensure field side instrument power supply is off.
• Ensure manifold to be isolated/closed before removing the instrument & air tubing.
• After removing the cable, the ends should be covered with insulation tape.
• Put LOTO or isolate fuse on the panel side before removing the cables.
• Field instruments, support, junction box and tubing must be dismantled & installed
properly using proper hand tools.
• Installation of field instruments shall ensure that operator & maintenance personnel
have easy access.
• Cable laying, Air tubing, glanding & Termination for new instruments.
• Ensure IA line was isolated before opening any tubing to dismantling.
• Ensure approval from O&M for cable to dismantling.
• Loop check must be done before Power ON the Instruments.
• Installation of field instruments must be inspected against installation detail prior to
installation.
• After terminating the cables, close the fuse & power ON the instruments.
• Verify the display is ON, in the field side. (Where applicable)

6.3. Working at Height (Scaffold & Platform Ladder)


• Ensure workers were done for working at height training before start work.
• Check the scaffold tag validity prior to climbing up and make sure it should be within 7
days.
• Must always wear a full body harness with self-retracting lanyard assembly when
using yellow tag scaffold platform and where required while working at height and
always 100% tie-off to the proper anchorage point.
• Tools must be tied with rated lanyard while working at height to prevent falling object
from height.
• Ensure to provide canvas on the platform while working at height to preventing from
falling object.
• Ensure proper toe boards / handrails are provided in the scaffold platform.
• Do not leave materials or tools on the scaffold platform at the end of work.
• inspection sticker on the platform ladder before using it.
• Must have an While using platform ladder, one person must hold the ladder while
climbing up/down.
• Must follow 3-point contact while climbing up/down from the ladder/stairway.

7. HEALTH, SAFETY AND ENVIRONMENT

7.1. Supervisor’s must ensure that method of statement (MS), Risk Assessment (RA)
7.2. Other necessary permits are available on site as well as preparations are checked.
7.3. Reviewed before allowing any work to commence.
7.4. Supervisors must brief all the workers with the contents all the method of statements, Risk
assessments during the daily toolbox meeting.
7.5. Ensure all workers and supervisors has attended and passed the Safety Induction
7.6. Course and other relevant training as prescribed by the Client.

8. HOUSEKEEPING / GENERAL

• Workplace housekeeping may be defined as activities undertaken to create or maintain an


orderly, clean, tidy and safe working environment.
• Effective housekeeping can eliminate many workplace hazards and help get work done
safely.
