APPOINT ATTY ADRIAN P. MAPALO, single, Filipino, with residence at Purok
Sacred Heart, San Isidro Talisay City, Cebu as my true and legal representative to act for
and in our name and stead and to perform the following acts:

1. To check the status of Civil Case No. 42238 for Declaration of nullity of
Marriage titled Peque v Peque in Branch 22 of the Regional Trial Court of Cebu City 7th
Judicial Region.

2. To obtain the necessary records of the above mentioned case in the

REGIONAL TRIAL COURT Branch 22 Cebu City in order to ascertain the status of the
aforementioned case.

HEREBY GRANTING unto my representative full power and authority to execute and
perform every act necessary to render effective the power to check the status of Civil
Case No. 42238 for Declaration of nullity of Marriage titled Peque v Peque in Branch 22
of the Regional Trial Court of Cebu City 7th Judicial Region and to obtain the records
necessary in order to ascertain the status of the aforementioned case, as though I myself,
have so performed it, and HEREBY APPROVING ALL that he may do by virtue hereof
