⁃ COLOSTOMY
⁃ The best site for colostomy is the mobile part of the colon which are
transverse colon and sigmoid colon .
⁃ in the sigmoid => left side abdomen
⁃ In transverse => right or upper abdomen .
⁃ Colostomy should be away from ribs , bone , umbilicus and scar .

⁃ ILEOSTOMY
⁃ on the right side

TYPE OF COLOSTOMY:-
1- End (Hartmann) ‫البروكزمال بارت يطلعونه برة البطن و يخلوله ستوما و الدستال يبقى بالداخل و يسدونه‬
Adv. :-
-comfortable to the pt.
-good care and followup
-less risk for infection

Dis:-
-nees laparotomy for anastomosis

2-double barrel )‫هنا البروكسزمال و الدستال اثنينهم يطلعونهم و يحطولهم ستوما (ممكن الستول يعبر بين الفتحتين‬

Adv:- easily anastomosis

Dis:-Incomplete diversion of the stool

3- colostomy with mucus fistula ‫هنا هم الدستال و البروكزمال يطلعونهم بس مو متصالت وحده يخلولها ستوما و‬
‫وحده يسونها فسچوال تطلع ميوكس‬

Adv:- Complete diversion of stool

Dis:-need more care

4-Loop ‫هاي يسونها باالميرجنسي‬

‫اللوب الي صايرله دايليتيشن( اابروكزمال) كله يطلعونه قبل ال يشيلون مسبب االنسداد و يحطوله ستوما‬

Adv:- fast

Dis:- need laparotomy for anastomosis

⁃ ‫ هم الكولوستومي يقسموها‬permanent ‫ و‬temporary

- ‫تكون دائمية اذا ماكو دستال بارت باقي حتى يسوله انستموسس مثال شايلين الركتم و االينص‬
- ‫ اما المؤقته‬:
There is distal part for anastomosis
Injury to the anal canal
I.O
In case of fistula between colon and bladder or between colon and vagina

-9-12 ‫ المؤقتة تبقى لمدة‬wks

Post op care foe pt with colonic ca.

1- fluid (maintenance and deficit )

Maintenance 2500-3000 G.W
deficit ‫ جان البيشت يطلع بايل الي بي‬، ‫حسب الكميه الي جاي يفقدها انطي الكميه و حسب شنو جاي يفقد انطي فلود‬
‫الكترواليت هواي فننطي رينگر‬

2-albumin: very important for healing

3-blood

4- vit C

5- antibiotic (3rd generation cephalo + mitrenodizole)

6-analgesic

7-DVT prophylactic
-enoxaparin 5000 twice daily , SC

8-physiothrapy for cough ‫جان عنده كحة‬

which occur due to wound and ansthesia

9-special care for wound, drainage , stoma

10- CT Scan best than us for hollow viscous, also benefit for drainage guidance.
