Two ways to conduct physical examination when giving


first aid:
1. Primary Survey - It is used when a victim is
unconscious and to find out and treat immediately life-
threatening conditions. It is usually done by checking
the consciousness: The ABC or DR ABC
 Ask the victim
 Check circulation of the victim
 Open airway
 Check breathing

2. Secondary Survey – It is used when a victim is


conscious or has revived. It aims to detect patient’s
condition. It includes history of the victim and checking
of vital signs. DCAPBTLS, Checking of vital signs,
SAMLE or SAMPLE PAIN.

DR ABC means:
D – Danger (check if the scene is safe)
R – Response (check for the patient’s response)
A – Airway (check the airway of any obstructions)
B – Breathing (check to see if breathing normally)
C – Circulation (check for signs of severe bleeding or
normal blood circulation)

DCAPBTLS means:
D – Deformity is a deformed part, especially of the body; a
malformation.
C – Contusions is a region of injured tissue or skin in which
blood capillaries have been ruptured; a bruise.
A – Abrasions is he process of scraping or wearing
something away.
P – Punctures is to pierce or perforate, as with a pointed
instrument.
B – Burns is a type of injury to skin, or other tissues, caused
by heat, cold, electricity, chemicals, friction, or radiation (like
sunburn)
T – Tenderness is pain or discomfort when an affected area
is touched.
L – Lacerations is a deep cut or tear in skin or flesh.
S – Swelling is the enlargement of organs, skin, or other
body parts. It is caused by a buildup of fluid in the tissues.

SAMPLE PAIN means:


S – Symptoms (By asking the question “What seems to be
bothering you?” a rescuer can get the answers from his or her
victim.)
A – Allergy (Rescuers can ask “Do you have any allergies I
should know about?” to determine the answer to this
question.)
M – Medication (A victim forgetting to take his or her
medication or a victim that carries a certain type of medication
(ex: Nitro for Heart Attacks), can help a rescuer determine the
problem. A simple question of “Are you on any medication?”
can help a rescuer obtain the information from the victim.)
P – Past medical history (finding out whether this has
happened to the victim in the past. If these symptoms are re-
occurring the victim can help by telling what the cause was in
the previous circumstance. A simple question of “Has this
happened to you before?” can aid in retrieving this information
from the victim.)
L – Last oral intake or Last meal eaten (Past meal time can
help in determine if the victim might be suffering from a
number of issues, with a likely scenario being low blood
sugar. In this circumstance the victim might need to increase
blood sugar. A good question to ask would be “When was
your last meal” or “Did you have breakfast/lunch/dinner?”)
E – Events prior to the accident (A good question in this
circumstance is “What were you doing before this
happened?”)
P – Period of pain (How long is the patient suffering in pain)
A – Area (On where particular are is the pain occurs)
I – Intensity (On what level is the pain the victim is feeling)
N – Nullify (On what part of the body does the victim cannot
feel)
