    The safety inspection found 4 issues at an HGU-II construction site operated by Dhaval Project: 1) Two workers were standing 10-12 meters high without fall protection while working on a grating and railing platform. They were removed from height and the entire crew was sent for counseling. 2) Two workers were doing insulation work without safety goggles. Goggles were provided and a penalty was imposed. 3) A reversing vehicle had no flagman to guide it. A flagman was subsequently provided. 4) A supervisor and two workers were found on a platform railing job without fall protection harnesses. The supervisor was counseled and penalties were imposed.
