지금까지는 이형기 교수님이 2교시 수업과 더불어 이 강의도 맡으셨는데, 올해 이 강의만 교수님이 바뀌셨습니다.
그래서 작년 필기와 족보를 참고하진 않았습니다.
올해 필기는 파란색, 필기자 첨언 및 별로 중요하지 않았던 교수님의 말은 회색으로 표시하였으며 교수님이 언급한 부분은
하이라이트치거나 네모 표시했습니다.
필기자 개인적으로 중요하다고 생각하거나, 교수님이 '기억해라', '기억하면 좋을 거다'라는 식으로 언급한 부분은 빨간색,
슬라이드에 중요한 게 많은 페이지는 특별히 오징어 스탬프를 붙였습니다.

Introduction:
작용 기전 및 저항성 발생 기전
1929 : Flemming이 세균 연구하다 세균이 죽는 걸 확인했다 - 곰팡이균(penicillium이라는 종)이 같이 배양되니 죽음
이에서 유래해 'penicillin' 탄생
Chain & Florey : 항균 작용 물질 정제를 위해 노력하여 정제 기술 발견
이 셋은 1945년에 이를 공로로 인정받아 노벨상을 타게 된다
History

β-Lactam Ring
이러한 구조 가지고 있으면 beta-lactam계의
항생제라고 칭한다
크게 4가지! β-lactam ring의 네모 구조에 쳐진 빨간
β-lactam ring은 모두 공통, 그 옆 구조들이 조금씩 다르다 네모는 강의록에 쳐져있던 것들입니다.
구조까지 기억하란 건 아니고, 옆이 달라짐에 따라 특성이 달라진다 정도만!

박테리아를 크게 그람양성, 그람음성으로 나누고
둘을 구분해야 하는 이유?
(나누는 기준 : 세포벽 구성 성분이 달라 나타나는 그람 염색 양상의 차이)
항생제들이 잘 듣는 게 다르다!
지금은 잘 안 와닿겠지만 나중에 감염병 치료에 있어
공통 : plama membrane / peptidoglycan 층
구분이 중요함! - 감수성 부분을 잘 기억해둬야 한다
그람양성 : peptidoglycan 층이 좀 더 두꺼움
그람음성 : outer membrane 존재
필기자 첨언 : 얘네는 아주
마아니 반복한 애들이라..
outer membrane이 하나 더 있음
-> gram negative에는 beta lactam이 작동하기 어려움
peptidoglycan 층의 역할
1)구조 유지
2)세포 내 osmotic pressure가 높은데, 이로 인해 세포가 터지지 않도록 유지

=PBP(penicillin binding protein)의 일종
항생제 저항성 발생 기전
1. Porins
Gram-negative bacteria have an outer cell membrane that restricts drug access to the peptidoglycan cell
wall to passage through porin channels peptidoglycan cell wall에 접근하는 것을 막아준다
The opening in porins is too small to allow diffusion of large molecules, such as vancomycin (resulting in an
inherent or intrinsic form of gram-negative resistance against glycopeptide antibiotics) 큰 것들은 들어가기 힘들다
Some gram-negative bacteria express porins having an altered pore structure that does not permeation of
smaller drugs (such as beta-lactams or carbapenems).
2. Pumps
Gram-negative bacteria can express P-glycoprotein-like ABC transporters in the inner plasma membrane
that function as efflux pumps to transport antibiotics out of the cell, thereby protecting the organism from
antibiotics having intracellular sites of action (e.g. topoisomerase or ribosomal subunits) (which is not a
mechanism terribly relevant for cell wall synthesis inhibitors). 약물을 자꾸 밖으로 퍼내는 역할-> 약이 작용하기 어려워진다

3. Penicillinases -> 항생제가 들어왔을 때 구조를 바꿔버리면서 항생제가 작동하지 못하게 해준다
The most common mechanism for drug resistance to β-
lactam antibiotics is bacterial synthesis of β-lactamases
(Hall et al, 2003).
Many bacteria synthesize beta-lactamases that degrade
beta-lactam antibiotics before they reach the cell wall.
Gram-positive bacteria that make beta-lactamase
excrete the enzyme into the extracellular space. Gram-
negative bacteria excrete beta-lactamase into the
periplasmic space located between the cytoplasmic
membrane & the outer membrane, where the cell wall is

근데 나중에 또 이 methicillin에 저항성 가지는 균 나타나서 치료에 어려움

3. Penicillinases
Penicillinase is a specific subtype of β-lactamase (the first β-lactamase to be identified in 1940).
It has a very limited specificity for inactivating penicillins. Resistance to penicillin due to
expression of “penicillinase” led to the development of “penicillinase-resistant” beta-lactams,
such as methicillin. Then some bacteria “struck back” and now “Methicillin Resistant Staph

aureus (MRSA)(or ORSA) is relatively common.
Extended-Spectrum Beta-Lactamases (ESBLs) are enzymes produced by several types of
gram-negative bacteria (E coli, Klebsiella, Enterobacter, Proteus) that endow bacteria with
resistance to all penicillins, cephalosporins, and monobactams, but do not affect sensitivity to
carbapenems (e.g. meropenem or imipenem)(CDC:Healthcare-associated Infections).

4. PBP : 항생제가 protein에 붙어야 작용하는데, 다른 모양으로 생겨 원래 붙어야 하는 자리가 변형됨
-항생제가 붙지 못하고 항생제가 작동 못함
-균마다 PBP 변형의 양상이 다름

약이 PBP에 붙어서 작용하는데, PBP 자체의 구조를 바꾸는 것이 있음 PBP 자체의 구조가 바뀌는 현상을 PBP 세번째라고
4. PBPs -> 작용을 잘 못하게 만들어준다
Bacteria can express a mutated PBP that still has enzymatic activity for cell wall
synthesis, but does not bind β-lactam antibiotics. These bacteria are drug-insensitive.
This is a mechanism of resistance used by S. aureus (MRSA), which expresses the
penicillin-insensitive PBP2a (expressed by the mecA gene. The mecA gene is found
within a transferable genetic element called mec. The mec containing mobile cassette
has spread rapidly through the S. aureus population via horizontal gene transfer and
selection from widespread antibiotic use (Bhattacharyya et al, 2015).
5. Peptidoglycan is absent
Some bacteria (e.g. mycobacteria)
lack a cell wall, and can multiply in the
presence of β-lactam antibiotics,
potentially resulting in serious
infection (e.g. atypical pneumonia).

여기까지 언급하심
•These agents are not effective against enterococci, and streptococci, and
their bulky side groups limits their ability to penetrate many other bacteria, and
hence they are generally only used to treat drug sensitive staphylococcal
infections (Hauser, 2013).
기억해야 할 건, MSSA/MRSA 다 이름에 Methicillin이 붙어있긴 한데, 지금은 항생제 감수성 테스트를 할 때 Methicillin
아닌 Oxacillin 사용하고 있다! 즉, Oxacillin으로 MSSA/MRSA (항생제에 듣는지 안 듣는지) 구별한다

•These penicillin analogs have an amino side group that increases their
hydrophilicity, allowing them to permeate through porins of some gram-
negative bacteria (e.g. E. coli, P. mirabili & Shigella spp.)
•Commonly used to treat listeria meningitis & enterococcal infections 여기에 사용할 수 있다
•They share the same vulnerability to beta-lactamases as the natural
penicillins, and hence are commonly combined with a beta-lactamase inhibitor,

e.g. beta lactamase를 다시 억제하면서 beta lactam의 구조를 유지하게
•ampicillin + sulbactam (Unasyn ®)
Augmentin 들어봤을 수도 있음!
•amoxicillin + clavulanic acid (Augmentin ®) 중이염 등 간단한 염증에 사용하는 약임
-listeria meningitis / enterococcal infection에 사용됨
-β-lactamase에 의해서 분해될 수 있다, 그래서 보통 β-lactamase inhibitor랑 같이 사용된다!
(거의 항상 붙어다닌다고 생각하면 됨)
•The side groups of these penicillin analogs allow for greater penetration into
gram-negative bacteria compared to aminopenicillins.
•These agents are also generally more resistant to many (but not all) gram-
negative beta-lactamases
•They are more active against Pseudomonsa aeruginosa
•These drugs achieve their fullest antimicrobial potential when combined with
β-lactamase inhibitors, and have activity against many aerobic gram-positive,
gram-negative bacteria, and nearly all anaerobic bacteria except C. difficile
•For example, the drug combination of extended spectrum penicillins with β-
lactamase inhibitors are useful in treating infections caused by many β-
lactamase producing strains of E. coli, Klebsiella spp, Bacteroides spp &
Enterobacter spp.
-Pseudomonsa aeruginosa 같은 균이 의심되면 사용
-broad-spectrum (aminopenicillin) 처럼 β-lactamase inhibitor와 같이 쓴다

-가면 갈수록 광범위한 균에 작용할 수 있도록 개발한다
즉, 뒷 세대로 갈수록 좀 더 스펙트럼이 넓게 작용한다
수술 시에 예방 요법으로 많이 사용된다

최종적으로 쓰는 형태를 보인다
