Confined Space Location: Arbutus Station 2097 W. Broadway

Confined Space Description: Confined Space at BS-01 Tail track/Minning area.

1.1 CONFINED SPACE ADMINISTRATOR


Confined Space Administrator: Andrea Harvey / Meinsen German Nahuel
Qualified Person: Risk assessment process and the drafting of confined space entry
procedures will be the responsibility of the Construction Safety Officer (CSO) or the
designated Supervisor for the activity. This work should be coordinated in close
collaboration with the safety team and the civil construction team.

It is essential that all persons involved in the activity are fully briefed on the specific risks
associated with confined space entry, as well as the safety protocols and procedures to be
followed in the event of an emergency. This includes a clear understanding of potential
hazards, such as lack of oxygen, presence of toxic gases, risks of entrapment or structural
collapse.

Adequate training on the use of personal protective equipment and atmosphere


monitoring tools should be provided to ensure a safe working environment prior to the start
of the activity.
Open and transparent communication between all parties involved is critical to ensure the
safety and well-being of workers in confined space environments. Any changes in site
conditions or operating procedures should be communicated in an effective and timely
manner to all team members.

A permit as detailed in BC OHS 9.13 is not required for this space because:
a. It does not have a high hazard atmosphere.
b. Isolation and lockout procedures are not required.
c. There is no hazard or entrapment or engulfment.

Although a permit is not required, a pre-entry assessment of air quality and slope
inspection must be maintained. Space occupancy must be monitored by the designated

shift standby person (attendance to account for safe exit in case of evacuation and at end
of shift).

1.2 ENTRY SUPERVISOR


The designated entry supervisors for the space are:
1. Day Shift (07:00 – 17:00): NAME PHONE NUMBER ______________________

The Entry Supervisor has the ongoing duty to ensure that entrants comply with the specific
requirements of the confined space entry safe work procedure, including:
1. All entrants have received appropriate instruction in the risks and safeguards
associated with the specific confined space.
2. Verification that testing and ventilating of the space has been performed and duly
logged, and, that conditions are within acceptable tolerances for entrance into the
confined space.
3. Verification that the Standby Attendant and emergency response equipment are
properly established at the entry to the confined space.

1.2.1 Authorized Entrants

Authorized entrants are those who have been trained and instructed in the hazard
assessment and safe entry procedures for this space by BSCGP. A list of authorized entrants
will be maintained by the standby attendant.

Authorized entrants have the ongoing duty to understand and follow the safe work
procedures for the confined space, including:
1. Take all mandatory training, including fit testing of personal respirators.
2. Know the hazards that may be encountered and the safeguards in place.
3. Use all personal protective equipment required for the space.
4. Do not deviate from safe work procedures without prior review and approval from the
Entry Supervisor
5. Maintain communications with the Standby Attendant, and immediately report any
unusual circumstances or symptoms.

6. Do not enter the confined space if unsure of any aspect of the work and/or
safeguards.
7. Follow emergency response protocols.

1.3 STANDBY ATTENDANTS


Each worker required to work inside the confined space will be assigned a partner. The
partner will be located on top of the confined space and maintain a visual on the worker
and be in constant verbal communication. Their partner will be responsible for
communicating with the worker, assessing their wellbeing, and initiating emergency rescue
if necessary.

1.4 CONTRACTOR USE OF PROGRAM


Contractors working in the area under the rebar will work under the BSCGP Confined Space
Entry Program and these procedures.

2 CONFINED SPACE COMMUNICATION


Workers will have direct verbal communication with their partner.

3 HAZARD CLASSIFICATION AND ASSESSMENT


Based on the Confined Space Hazard Assessment the space is classified as Low Hazard.

3.1.1.1 Low Hazard

A Low hazard atmosphere means an atmosphere which is shown by pre-entry testing or


otherwise known to contain clean respirable air immediately prior to entry to a confined
space and which is not likely to change during the work activity, as determined by a
qualified person after consideration of the design, construction and use of the confined
space, the work activities to be performed, and all engineering controls required by the
regulations.

4.1 CONFINED SPACE ENTRY REQUIREMENTS


Authorized entrants are to be knowledgeable with this procedure.
1. No worker may enter a confined space until:
a. Workers have received instruction on the risks and safeguards associated with the
entry and the work activities to be performed.
b. Workers have donned the personal protective equipment appropriate to the hazards
of the confined space.
c. Pre entry air monitoring has been carried out and continuous air monitoring is set up
for when workers are in the confined space, this also applies for when workers leave
and re-enter the space. This will be done by a trained and competent person.
2. All workers entering a confined space must be in continuous communication with their
partner outside the confined space. The partner will maintain visual and verbal contact
with the worker.
3. Anchor Point: good Anchor Point will be established. Worker will be tied off while
performing task.

4.1.1 Pre Entry Procedures

1. The Entry Supervisor will ensure that all safety equipment is present and in good working
condition prior to starting the work area.

- Conduct a risk assessment before starting work in the confined space.


- Designate a supervisor responsible for the work and rescue equipment.
- Verify that all personal protective equipment (PPE) is available and in good condition.
- Check the availability and proper functioning of rescue equipment, including the
crane, stretcher and rescue basket.
- Establish a clear communication system within the confined space, such as radios or
visual signals.

2. The Entry Supervisor will ensure that all personnel involved in the entry are familiar with
the procedures to be used (including the specialized rescue procedures), have a clear
understanding of the role and duties of the partner, and have had an opportunity to ask
any questions, or satisfy any concerns, prior to the entry.
3. Workers must complete a field level risk assessment (FLRA) prior to starting any activity.
The task supervisor is responsible for overseeing and approving the confined space

entry procedure, as well as ensuring that workers complete the atmospheric monitoring
and sign the document. Once completed, the document should be promptly forwarded
to the CSO for review.

5 PERSONAL PROTECTIVE EQUIPMENT


All entrants must use/wear both general and task specific personal protective equipment
during confined space entry, including but not limited to:
 Steel toed work boots, hard hats, safety eyewear and high visibility vests,
 Clothing adequate to protect the entrant from the hazards of the work, including long
pants, gloves, hearing protection, face shields, fireproof clothing etc.
 All specialized personal protective equipment related to confined space entry _ D ring
Body harness and a spreader bar.
 The following equipment must be in place before work commence:

1.) Speeder bars


2.) Ventilation
3.) Air monitor
4.) Access leader
5.) Air horns
6.) Drings harness
7.) Davit Arm with a spread bar attached to the arm.

5.1.1 Extraction Equipment

A) Davit Arm & DEP BOX Lift:

The Davit arm equipment will serve as the primary method for extracting a worker
from the confined space. It will be stationed within the confined space and a spread
bar always will be attached to it. Workers must wear the D-ring harness at all times
within the designated area.
The spine board, stretcher, first aid jump kit, and oxygen will be brought to the work
area when the first aid attendant initiates the rescue plan. Subsequently, upon
initiation of the rescue plan, the first aid jump kit and oxygen will be promptly

placed inside the DEP BOX, which will already contain a stretcher. The DEP BOX will
then be rigged and lowered into the work area inside the excavation by designated
emergency personnel.

If the DEP box cannot be lowered close enough to the work area, the spine board will
be removed from the DEP box and placed near the injured worker. After securing the
worker onto the spine board and stretcher, a team of 3-4 individuals will lift the
stretcher back to the DEP box. Then, the DEP box will be rigged out of the excavation
for ambulance access.

6 EMERGENCY RESPONSE AND RESCUE


A specialized emergency plan has been developed for the work.

 In case of emergency, alert the supervisor or rescue team using the established
communication system.
 Assess the situation and determine the most appropriate rescue method, whether
using the Davit Arm, Tower Crane and DEP Box stretcher or a Tower Crane/rescue
basket lift.
 Coordinate the rescue according to the previously established plan, ensuring the
safety of both the rescuer and the trapped worker.
 Perform the rescue with care and following established safety procedures.

6.1 EMERGENCY RESPONSIBILITIES


All employees with emergency responsibilities related to the confined space, must be
informed of and understand their responsibilities.

6.1.1 Partner

In the event that an emergency rescue is required, the Partner will:


1. Immediately contact the First Aid Attendant – via air horn, cell phone or radio. If via radio
or cell phone, the partner will tell them:
1.1. The nature of the emergency (fire, medical, injury)

1.2. The Davit arm equipment will serve as the primary method for extracting a
worker from the confined space. It will be stationed within the confined space
and a spread bar always will be attached to it.
2. If necessary, inform other entrants of the emergency and instruct entrants to
immediately evacuate the confined space (one long continuous blast of the airhorn)
3. Inform the Entry Supervisor that an emergency rescue is required.
4. If necessary, contact 911 and report that an incident has occurred, providing:
the exact location: 2097 West Broadway
a. the nature of the emergency, (patient injured, medical emergency etc.)
5. Direct other workers and entrants to shut down any equipment stop work and evacuate
the excavation until the emergency is managed.

6.1.2 Construction Safety Officer / OFA2-3

In the event that an emergency rescue is required, the CSO/OFA 2-3 will:
1. Immediately report to the confined space are with the necessary equipment and/or
activate the emergency response plan including delegating a designated person to
bring the Level 3 First Aid Kit to the confined space if necessary.
2. Treat injured workers within the scope and limitations of their training.
3. Supervise the patient who has been packaged onto the spine board and
stretcher basket within the confined space as it is rigged and secured for
extraction by the tower crane. Ensure proper monitoring during the extraction
process and oversee the safe removal of the injured worker from the excavation
while inside the stretcher basket.
4. Pass injured workers care off to emergency services.

6.1.3 Entrants / Co-workers / Helpers

1. Communicate emergencies to the standby attendants.


2. Evacuate the space as quickly and safely as possible when required.
3. Assist the CSO/ OFA in carrying an injured worker out in the basket stretcher.

6.1.4 Entry Supervisor

The Entry Supervisor has the overall control of the scene and directs the actions to
coordinate the resolution of the incident until the arrival of the emergency response
agencies. On notification of the incident, the Entry Supervisor proceeds to a point near the
confined space and directs the following actions:
1. Inform the workers to immediately cease all work activities and shut down all
equipment.
2. Provide resources and direction as required to facilitate the removal of the patient from
the confined space. For example:

a. Dispatch worker to await arrival of emergency response agency personnel and


direct them to the incident scene.
