AFFIDAVIT

I, LESLIE LEGARDA SO ALIHUDDIN, of legal age, Filipino, married

and a resident of 1610 D. Paso St., Bagumbayan, Taguig City, Metro Manila,
Philippines after having been sworn to in accordance with law depose and state:

1. That I am the owner-buyer of parcels of land embraced by Transfer

Certificate of Title Nos. T-219276 and T-112498 of the Registry of Deeds of
Bulacan (Annexes “A” and “B” of Petition).

2. That the Owner’s Duplicate Copy of Transfer Certificate of Title Nos. T-

219276 and T-112498 were delivered to me upon the execution of the deed of sale
by the seller. Copy of the Deed of Absolute Sale is hereto attached as Annex “A”.

3. That the original copy of Transfer Certificate of Title Nos. T-214276 and
T-112498 on file with the Registry of Deeds of Bulacan was lost or destroyed to
fire.

4. That no deed or other instrument affecting the property had been

presented for registration;

5. That the owner’s duplicate certificate of co-owner’s duplicate is in due

form without any apparent intentional alterations or erasures;

6. That the certificate of title is not the subject of litigation or investigation,

administrative or judicial, regarding its genuineness or due execution or issuance;

7. That the certificate of title was in full force and effect at the time it was
lost or destroyed;

8. That the certificate of title is covered by a tax declaration regularly issued

by the Assessor’s Office; and
 9. That the real estate taxes have been fully paid up to at least two (2) years
prior to the filing of the petition for reconstitution.

10. That the Owner’s Duplicate of Transfer Certificate of Title Nos. T-

219276 and T-112498 (Annexes “A” and “B” of Petition) is submitted as basis for
administrative reconstitution pursuant to Republic Act No. 6732.

11. That I am executing this affidavit in connection with my petition for

administrative reconstitution of the original copy of Transfer Certificate of Title
Nos. T-219276 and T-112498 of the Registry of Deeds of Bulacan.
