5. Reasons for this Request :

This proposed Final Variation Order involves increase and decrease in the quantities
of original work items in the contract which is based on the approved As-Staked Plan
to cover the changes which incorporate revisions/alterations deemed to suit with the
present/actual field conditions resulting to overrun and underrun of quantities.

The proposed Final Variation Order involves mainly the following:

9. The quantities as presented in the Itemized Cost of Revision were reviewed/checked

by the Construction Section based on the approved As-Built Plan, Detailed Technical
Justifications and Detailed Quantity Estimate.

10.Per review, the unit price of work items with increase in quantities and covered by
the proposed Change Order were found similar or exactly the same to those in the
original contract.

11. In view of this, we respectfully recommend the approval of Variation Order

No. 01 in accordance with Department Order No. 148, Series of 2018 known
as “Implementation of the Document Tracking System (DoTS) for Civil Works”
and based on the provisions of Annex E of the 2016 Revised Implementing
Rules and Regulations (IRR) of Republic Act (RA) 9184 states that:

1.2. A Change Order may be issued by the implementing official to cover any
increase/decrease in quantities of original work items in the contract, the
herein request is being submitted for favorable consideration and approval.

In view of the foregoing and as per favorable recommendation of the Project

Engineer, the herein Final Variation Order for the contract of SUNWEST CONSTRUCTION
AND DEVELOPMENT CORPORATION in connection with the implementation of the
above subject project, is respectfully submitted for approval.
