Temp nya ay 36.8°c, pr ay 130, rr nya ay 40 tapos yung oxygen saturation ay 98%.
Weight is 20 pounds at height ay 29 inches. Wala ding lagnat
si baby. Tapos wala din naman daw any allergy sa gamot at pagkain.

Christine: Hello Ms.darra inform lang kita na may bago tayong admission sa room 204,
1 yr old male, ang diagnosis nito ay ano undescended testes at for surgery ito tomorrow
ng 8am tapos dadalhin na namin dyan ang patient. 

Darra: Okay thankyou

Christine: Ahm.. Ms.darra nadala na namin sa room 204 si patient, time od admission
ay 3 pm. Mag eendorse na ko. Ito ay lalake, si Mr. Zhayden Delos Reyes, 1 year old,
taga San Pascual, wala syang existing na ibang sakit ang complain lang nito ay hindi
daw bumababa yung itlog ng bata sa left side ng ari. Tapos ang diagnosis naman ni Dr.
Coloz ay ano undescended testes. Wala naman syang lagnat, di rin naman nagsuka.
Wala ding any allergy sa pagkain at gamot. Nakuhanan na naman namin si patient ng
VS, lahat naman ay normal. Sa family history naman meron daw undescended testis sa
part ng husband nya. Di pa naman sya naaadmitt dati ngayon pa lang daw. Tapos sa
doctors order ang patient ay admitted under service of Dr. Lira to ROC. Nasecure na
namin ang consent for admission at consent for surgery tapos naitawag na din ni Dr.
Coloz kay Dr. Rosal for anes at Dr. Cloda for assist at na acknowledge na naman ang
referral okay na. Ang CP clearance naman ay care of na ni Dr. Smirz tapos imonitor
daw ang VS q4 and record. Pati ang I & O at TPR q shift. Tsaka AP aware daw yan for
this admission. For the IV ng patient naman D5 LR 500 cc 35cc/hr to start naman ng

every 6 hours for 4 doses. Nakuhanan ko na ng VS every 15 mins hanggang maging
stable sya at ang initial VS nya ay T: 36.4, BP: 70/80, RR: 17cpm, PR: 128 O2: 98%
even the I&O recorded na. Wala naman akong napansin during my shift na bleeding or
respiratory distress sa bata. All goods naman.

sa paghinga, lagnat basta po kahit anong masmaang sintomas. Pwede nap o siya
umuwi. Pero incase po na meron, ipagbigay alam niyo po agad saamin at nasal abas
