BRGY. HUBASAN CALBIGA, SAMAR HELD ON MONDAY, APRIL 19, 2021 AT 8:00 IN

RESOLUTION NO. 18

*A RESOLUTION FOR THE ESTABLISHMENT OF BARANGAY REFERRAL

REHABILITATION DESK OF BARANGAY HUBASAN, CALBIGA, SAMAR*

WHEREAS, Section (3) (a) of Dangerous Drug Board Regulation No. 2 Series of 2016,
mandates the whole process of voluntary surrender to authorities shall be documented by
the duty officer of the barangay where the individual surrendered;

WHEREAS Section (6) (a) No. 3 of the Dangerous Drugs Board Regulation No. 05 Series of
2017 provides for the basis in the organization of house cluster with designated cluster
leader in each barangay;

WHEREAS Section (a) (7) of Dangerous Drugs Regulation No. 3 Series of 2017 further
provides of the establishment of Rehabilitation Referral Desk in every barangay to endorse
drug dependents to the duty authorized representatives of DDB pursuant to Article 3 of RA
9165 and other pertinent issuance of the board;

WHEREAS, considering above premises and therefore on motion of Hon. JONATHAN C.

CABIGAYAN, Committee Chair on Peace and Order/BADAC Vice Chairperson, duly
seconded by Hon. SONNY O. CALUTAN to approved of which motion has earned the
approval of all members present to recommend the establishment of Barangay Referral
Rehabilitation Desk of Barangay Hubasan, Calbiga, Samar.

Section 1. Establishment of Barangay Referral Rehabilitation Desk.

A. I, hereby designated the Barangay Health Worker, the Officer of the Day on duty,
as the Barangay Duty Officer.

B. Roles and Responsibilities,

1. Man the Rehabilitation Desk.

2. Conduct an interview to solicit personal information from surrenderers.

3. Immediately inform the Punong Barangay in case the surrenderers is included in

the target list, wanted list and watch list personalities of the law enforcement agency such
as, but not limited to PDEA and PNP.

4. Refer or endorse drug dependent to the MADAC for screening using the Alcohol,
Smoking and Substance Involvement Screening Test or (ASSIST);

5. Log and maintain record book containing all personal information of all
surrenderers;

6. Ensure confidentiality of all information and surrenderers;

7. Monitor progress of surrenderers;

8. Perform other duties and functions as may deemed necessary.

RESOLVED, as it is hereby resolved to forward copies of this Resolution to the MADAC

through the PNP for their favorable consideration and final endorsement to the PDEA
Oversight Committee, Palo, Leyte, requesting issuances of a Drug-Unaffected Certification,
copy furnished the DILG and MSWD and Sangguniang Bayan for their information,
reference and favorable action.
