AFFIDAVIT

I, SILVANO N. GARSUTA, JR., Filipino, of legal age, married and a resident

of 34-1 Sikatuna Street, Tagbilaran City, after being duly sworn to in accordance with
law, do hereby depose and say that:

1. I am a Geodetic Engineer with Professional Regulations Commission

Registration No. 4711, dated September 3, 1997 and PTR license No. 3842119, dated
January 4, 2007;

2. I have conducted a survey of Lot No. 4280-A, Psd-40928 under Transfer

Certificate of Title No. T-49080 located at Poblacion, Tagbilaran City owned by spouses
CARINO A. NAVEA and VIRGINIA MAYOL;

3. The above-mentioned property is located at a distance of 7.73 METERS from

the center of the existing road (Graham Avenue). A copy of the sketch plan is hereto
attached as Annex “A” hereof;

4. I execute this affidavit to attest to the truth of the foregoing, to the application
for a building permit of the above-named lot owners and for whatever legal purpose this
may be worth.
