Incident:

Approximately at 17.45 hrs on 31st July 2021 during premove of Rig 38, the Truck Oman crane
operator was moving the crane near the border of location. The boom of crane was not
lowered and lifting slings were also attached to the hook. When the crane stopped near the
vicinity of the overhead Powerline, the slings swung Forward and contacted with the power
line. crane boom on and crane boom struck with overhead power line. The operator
immediately lowered the boom after observing sparks and notified to PDO Electrical team. No
personal injury.

Findings:
- Road survey was done before rig move by truck Oman but nothing mentioned regarding the
risk of overhead lines
- TBT of Rig Move was done at old location however Crane operator did not attend as he
arrived late.
- Banksman was available but he was busy for latching Weatherford container from the truck.
- Crane Boom was not lowered and slings were not removed during the Movement of the
crane.

Learnings:

- Always ensure crane boom is lowered and slings are removed prior to move the crane.
- Ensure dedicated banksman for any critical task during rig move
- Ensure that road survey of rig move cover all out of sight hazards.
- Ensure all hazards are discussed with Rig move team prior to Rig move.

Always ensure crane boom is lowered while moving in the vicinity of power lines

Incident:

During years 2020-2021 YTD ~ 10 Fines / Observations received from


Environment Authority due to Sewage overflow occurred at different camp
locations occupied by UWD units which resulted in environmental damage

Findings: Sewage overflow with shrubs


• eCCC environment area rated as green
• Poor compliance to Sewage Overflow Prevention SOP
• Location pre-move checks - accepted with overflow already in place (cont'd
without fixing the issue)
• Fines were issued by Environment Authority – why they were not reported by
PDO/Contractor

Learnings:
• Ensure compliance to simplified environmental requirement
• Ensure Compliance to Overflow Prevention SOP
• Increased supervision by site leadership (Accountability and Responsibility) Camp Sewage cleaned
• Ensure compliance to location pre-move checks - report any abnormalities
• Office support: ensure the environment compliance is checked during regular different engagements
• Report all incidents related to sewage in PIM
