Good day! I Analyn A. Morcilla, currently enrolled at MinSu Calapan City, Calapan
Campus, a 2nd year student of Bachelor of Science in Tourism Management, residing at
Balatero, Puerto Galera, would like to inform your good office that I’m not feeling well
right now because of fever. However, I hereby authorize Ms. Mary Jane Pelaez to claim
my requested Certificate of Grades (COG) last August 30, 2022.
I apologize for the inconvenience this may cause and I appreciate your consideration in
