Task 3
Hold a discussion with a supervisor regarding your wellbeing.
BENCHMARKS
 create and record an action plan to remedy issues with the supervisor
 reach an agreement with the supervisor
 notate a time for when the plan needs to be reviewed and evaluated in a calendar or diary

Submission requirements:

1 x table or word document outlining the negative wellbeing factors discussed at your meeting in
TASK 2. You will need to list the PROBLEM you discussed (what the problem is and why it’s a
problem), the proposed SOLUTION arranged with your supervisor and when you are going to
REVIEW your plan of action, including the date/time and what you want the outcome to be.
