Contract Agreement

THIS AGREEMENT, made and entered into this ___ day of December, 2022 by and
between

The MUNICIPAL GOVERNMENT OF TERESA with office address at

Municipal Hall, Corazon C. Aquino Avenue, Barangay Poblacion, Teresa,
herein represented by the Municipal Mayor, Hon. Rodel N. Dela Cruz,
whose authority is evidenced by the attached Sanggunian Bayan
Resolution, marked as Annex “A” and made integral part hereof,
hereinafter referred toto as the “Entity”;

-and-

The PLUME SERVICE STATION, a business entity, registered under the

laws of the Republic of the Philippines, with business address at Bulak,
Bagumbayan, Teresa, Rizal, herein represented by its owner, ANGELO
GABRIEL B. SAMIA, hereinafter referred to as the “Contractor”.

WITNESSETH:

WHEREAS, the ENTITY conducts various activities and meetings in the implementation
of its programs and oftentimes need fuel for the municipal vehicles;

WHEREAS, being one of the nearest fuel supply stores in the vicinity, the ENTITY is
desirous for the CONTRACTOR to supply the requirements available in its store for
various activities of the ENTITY from December 10, 2022 to December 9, 2023;

WHEREAS, the ENTITY shall notify the CONTRACTOR with the Delivery Trip Ticket
including the liters needed;

WHEREAS, the ENTITY shall pay the CONTRACTOR after the activity or before the
end of each calendar month, whichever is more applicable;

WHEREAS, the CONTRACTOR provides the Sales Invoices for the week for billing
purposes;

WHEREAS, in consideration of the payments to be made by the ENTITY to the

CONTRACTOR as hereinafter mentioned, the CONTRACTOR hereby covenants to
supply fuel needs of the ENTITY for the duration of this contract;

WHEREAS, the ENTITY hereby covenants to pay the CONTRACTOR in consideration

of the execution and completion of the delivery, the Contract Price or such other sum as
may become payable under the provisions of this Contract at the times and in the
manner prescribed by this Contract;

IN WITNESS whereof, the parties hereto have caused this Agreement to be executed
the day and year first before written.
