JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

married, of legal age, and a resident of Prk XXX, both being duly sworn to in accordance with
law, depose and say that:

1. We personally know XXX who was born on XXX both Filipino citizens;

2. We know the aforementioned facts because long before XXX was born, we were her
parent’s neighbors in Leyte and a close family friend up to this day;

3. That XXX has been using the said birth name and birth name in all her documents and
transactions;

4. We are not related to XXX either by affinity or consanguinity;

5. We are executing this joint affidavit to attest to the truthfulness of the aforementioned
facts.
