With reference to the incident overhead power line damaged at km 115 It was concluded through a
detailed investigation that you didn’t comply with the site safety procedure, load securement
requirements, safe driving procedure and did not fully comply with your assigned job & responsibilities.

Thus, with the reference to (Standard Penalties for violations), SINOPEC MIP PKG 17 Management has
decided to remove it from MIP PKG 17 project site.

Your cooperation will be highly encouraged for HSE Duty Compliance on the worksite, which will
promote a good safety culture in the Organization.
