Greetings of peace and love!

I, HAZEL JUNE M. FABILLAR, Teacher II , currently teaching at Capipines

Elementary School, Bindoy District 2, Bindoy Negros Orienal. I heartily express my interest in
applying for transfer of teaching station nearest to my residence.

Kindly, I wanted to request from your good office to shift my teaching assignmemt from
Capipines Elementary School, Bindoy Negros Oriental to Tampocon 1 Elementary School
Ayungon 1 Distrct. for valid reasons stated herein based on the DepEd order No. 22, s. 2013.

1. That I am teaching in my present station for four (5) years and 4 months with utmost
dedication.

2. The proximity of my recent job location is too far from my home and I am a nursing mother of
a one year old baby which needs my attention and care.

3.My health condition. I have been examined that I have Cardiac Dysrhythmia and Hypertension
Stage 1.

4. I will be more productive and effective teacher if my station is near to my residence.

I do possess effective teaching skills and still eager to learn more with regards to my profession.
It is my fervent hope and prayer that this humble request will be given appropriate action from
your office. It would be beneficial for me and my children’s well-being.
