(RE: Demolished House)

I, TEODORO T. TAÑAQUIN, Filipino citizen, of legal age, married with

residence and postal address at Blk 76 L 25 12th-21st St., Barangay 183, Villamor,
Pasay City, after duly sworn to in accordance with law, depose and say:

That I am the rightful owner of the house and lot located at Block 47 Lot 25,
12th-21st St., Barangay 183, Villamor, Pasay City;

That on May 17, 2021, my carpenters/contractors started to demolish my old

house and ended on May 25, 2021;

That there is already an existing new house at the above-mentioned address;

That I am executing this affidavit to inform the proper authority of the truth of
the foregoing facts for whatever legal purpose it may serve.

In Witness Whereof, I affixed my signature this 20th day of October 2022 in

Parañaque City, Metro Manila, Philippines.
