We, LEONARDO P. CARAYAO and ARNOLFO P. CARAYAO, both
Filipinos, of legal age, and with residence address at Purok 3, Lublub, Alfonso Castaneda, Nueva Vizcaya, after having been sworn under into law, hereby despose and state that:
1. We are the siblings of ROMEO P. CARAYAO (hereinafter
referred to as “the decedent”), who died on 11 September 2019 in Makati City, Philippines.
2. The decedent was legally married to his wife, GILDA
O. CARAYAO, and together they had two (2) daughters, DARYL O. CARAYAO and MARIBEL AILYN O. CARAYAO, who are both currently residing at 2268 Pasigline Street, Sta. Ana, Manila.
3. Due to irreconcilable differences between the
spouses, both Romeo Carayao and his wife Gilda Carayao were separated sometime around 1988 until the former’s death in 2019.
4. During their separation, Romeo Carayao married
CRISTINA M. SUBIBI until the former’s death. In addition, Gilda Carayao co-habited with her partner RODOLFO DEO, JR., until the latter’s death in 2018.
5. Romeo Carayao and Gilda Carayao did NOT have any
children from their respective partners.
6. Despite the aforementioned separation and the
subsequent marriage/co-habitation of Romeo Carayao and Gilda Carayao, their daughter MARIBEL AILYN CARAYAO still depended on her father, the decedent, for financial support.
7. We are attesting the truth of the foregoing facts for
the purpose of the application of Romeo P. Carayao’s Death Benefits/Claims before the Social Security System (SSS) and for whatever legal purpose this may serve.
Further, affiants sayeth naught.
