TIME LEARNING TASK
Preliminaries
 Wake up, make up your bed, eat breakfast and get ready for an awesome day!
 Have a short exercise/meditation/bonding with family.
Introduction (What I Need to Know)
 As the lesson starts, tell the learner that in this module, he/she will know how to fill out forms
accurately and appreciate the importance of filling out forms.
 Have the learner get ready to explore his/her module and prepare his/her answer sheet/s.
Pre assessment (What I Know)
 Instruct the learner to answer the first activity on page 2
Let him/her fill out the form below and complete it by using the possible answers found in the
box. Use the form provided.
Remember: If the learner needs assistance, guide him/her in answering the activity.
PROCEDURE
1. Review/Motivation (What’s In)
 This part of the module has three different forms to identify. Have the learner answers the
activity on page 3.
 For this activity, examine the forms below and then identify each. Pick your answer from the
choices inside the box. Write it on your answer sheet.
Remember: This activity will prepare the learner for the current lesson. Help your child as he/she
answers the activity.
2. Initial Activity/Discovery (What’s New)
 Have the learner answers activity on page 4.
For this activity, let the pupil examine closely the completed (filled out) forms and the required
information that was supplied in each form. Using a Venn diagram, write down the similarities
and differences of the forms based on the required information in filling it out.
Remember: Guide the learner in answering the task. Parents may remind the learner of what he/she
needs to do in this activity.
3. Discussion of the Topic (What Is It)
 Take time to read pages 5 and 6. Try to understand the information about deposit and
withdrawal slip and the steps on how to fill out each form.
 Remember: Instruct your child to read the content carefully. Help your child discovers the
lesson by discussing and giving explanations about the content.
4. Enrichment Activities (What’s More)
 This part of the module will determine if the learner understands the lesson. Have the learner
answers activity on page 7.
Let the pupil fill out the withdrawal slip using the suggested information found in the box. Use
the form provided.
Remember: Remind him/her to read the directions carefully. Let your child practice answering the
activity on his/her own.
5. Generalization (What I Have Learned)
 The learner is expected to know particular forms, how to fill them and appreciate their
importance.
6. Application (What I Can Do)
 The learner must apply the knowledge he/she has learned about the topic by answering the
activity.
 On page 9, read the selection about Ana and her Grandma. Help your friend Ana, by completing
the withdrawal slip for her grandma. Use the form provided. Answer the activity on page 10.
Remember: In this part, allow the learner to answer the activity by himself/herself. Let the child works
independently.
7. Assessment (Post assessment)
 Tell the learner if he/she understands the lesson by answering activity on page 11.
 You have a savings bank account and you want to put an amount of 2, 000 pesos into it, with the
following cash breakdown: two 500 pesos and ten 100 pesos. Using the bank account number
0344-5555-22, complete a cash deposit slip. Fill out the form provided to you.
 Remember: This part also requires the learner to answer the activity on his/her own. Advise
your child to do his/her best in answering.
8. Additional Activity
 Have the learner answers the additional activity on page 12.
 Let him/her complete the school form shown. Use the form provided.
Remember: At the end of this module, parents may give praises or rewards to his/her child for the job
well done.

MODE OF DELIVERY

Parent or guardian shall return the finished module to the teacher/adviser on the given scheduled date of retrieval.
