‫إستخدام االختصارات الطبية والرموز التشخيصية‬

‫الغرض‪:‬‬ ‫‪‬‬
‫تجنب سوء فهم اإلختصارات الطبية والرموز للحفاظ علي أمان وسالمة المريض فعاد ًة ما يتم استخدام الرموز‬
‫واالختصارات لضغط الكثير من الكتابة في مساحة صغيرة‪ .‬قد يتسبب هذا في سوء التواصل بين المتخصصين في‬
‫الرعاية الصحية واألخطاء المحتملة في رعاية المرضى‪.‬‬

‫السياسة‪:‬‬ ‫‪‬‬
‫وضعت المستشفى سياسة وإجراءات للرموز‪ 5‬واالختصارات المعتمدة وغير المعتمدة وف ًقا لنطاق‪ 5‬خدمة المستشفى ولغة‬
‫االتصال الرسمية المعتمدة داخل المستشفى والتي تتناول على األقل ما يلي‪:‬‬
‫أ) قائمة الرموز‪ / 5‬االختصارات المعتمدة ‪.‬‬
‫ب ) قائمة الرموز ‪ /‬االختصارات‪ 5‬التي يجب عدم استخدامها‪.‬‬

‫إجراءات العمل‪:‬‬ ‫‪‬‬

‫‪-‬يقوم رؤساء األقسام الطبية باختيار‪ 5‬لجنه طبيه العداد قائمة بالرموزواإلختصارات‪ 5‬المسموحة والمعتمدة داخل‬
‫المستشفى وتعرض علي اللجنة الطبية الموسعة لإلعتماد ثم يقوم فريق الجودة بإصدار القائمة و توزيعها‪ 5‬علي‬
‫األقسام‪5.‬‬
‫‪-‬يقوم فريق الصيادلة بإعداد قائمة بالرموزواإلختصارات الممنوع تداولها داخل المستشفى وتعتمد‪ 5‬في لجنة الصيدلية‬
‫واألدوية العالجية ويتم توزيعها علي األقسام ‪.‬‬
‫‪-‬عند كتابة األوامر العالجية يجب أن يقوم الطبيب بكتابة إسم الدواء كامل وتركيزه وشكله الصيدلي و الجرعة‬
‫وطريقة اإلستخدام والتكرار‪ 5‬ومدة العالج وكتابة إسم المريض ووزنه وسنه وإستخدام‪ 5‬فواصل بين األصفار‪.‬‬
‫‪-‬اليجب إستخدام" اإلختصارات حتي المسموحة في حاالت‪:‬‬
‫*اإلقرارات المبنية علي معرفة المريض‪.‬‬
‫*أي نماذج طبية يتلقاها المريض أو ذويه‪.‬‬

‫* يتم توجيه تنفيذ قائمة اختصارات الممنوعة لألدوية من خالل مراجع موثوقة ‪ ،‬على سبيل المثال ‪ ،‬قائمة معهد‬
‫ممارسات‪ 5‬األدوية اآلمنة (‪ )ISMP‬وتشمل على األقل ما يلي‪:‬‬
