 Issue: Installing rebar and concrete casting for 12 pile caps as mention in Location was done without
inspection; on informing. Cast in early morning (1:25 to 4:30) on Jan 17, 2019, contractor finished rebar
installation in the midnight and instantaneously concreted.
Status: MAM disapproved for the 12 pile caps concrete casting.
