ACTIVITY COMPLETION REPORT

I. Title: Homeroom PTA Meeting

II. Location : Tabango Senior High School
III. Date : June 14, 2018
IV. Introduction:
In view of the mandated of the Department of Education, Tabango Senior High School open the
school year 2018-2019. Population of the school was growing, which necessitated the conduct of PTA meeting to
formally inform the parents, the policies and guidelines of the school.
Authorized contribution, election of new set of PTA officers and some important matter to be discuss.
V. Accomplishment
After the election and proclamation of a new set of HPTA officer. The undersigned thought the way
to help achieve a quality teaching and learning inside the classroom. Thus, the idea of organizing a project for 11
ABM, they will collect an amount for the wall fan.
At the end of the meeting, the parents agreed that the elected officers will contribute 100 pesos and for
the non-officers will be 50 pesos for the said project. The deadline of the contribution will be on last week of July.
VI. Problem Met
Out of 33 students enrolled in 11 ABM class, only 19 parents were present during HPTA meeting.
VII. Recommendations/ Suggestions
Parents/ guardians were encouraged to open up their concerns and suggestions in order to give an
immediate action.
VIII. Good Practices Learned
The parents/ guardians rights and privileges are being recognized. Continue to motivate the
stakeholder to participate for the wellness of their students.
IX. Sustainability Plan
Continue to encourage and give importance the effort and cooperation of the stakeholder.
X. Replication Plan
Realization of the said project will promote the effectiveness and conducive environment for teaching
and learning process.
