Dx: Guillain Barre syndrome
C/C: pt c/o weakness in her hands and legs (legs>arms) causing her difficulty in walking and other daily activities. Pt also c/o
easy fatigability.

HPI

4 weeks ago, pt consulted a doctor because she was bothered of having coughs and colds lasting for 2 weeks already. She was
diagnosed with upper respiratory tract infection and was given medications (name of the medications unrecalled).
3 weeks ago, pt’s coughs and colds had improved, but patient started to feel numbness and tingling in her feet. She did not do
anything about this abnormal sensation on her feet. After 3 days, she felt also the numbness and tingling sensation on her legs
and thighs, but still did not do anything about this.

2 weeks ago, pt noted that the numbness became progressively worse and she felt it in her hands and fingers. She also started
to feel weakness on her both legs causing her to have difficulty in walking. She noticed that she usually trips when walking.

1 week ago, pt suddenly fell as she walks going to the kitchen. Her mother forced her to seek medical help already because of
this. Pt was then immediately asked to be admitted in the hospital for further monitoring and treatment.

At present, pt was referred to physical therapy for further evaluation and treatment. Pt is still admitted in the hospital. She cannot
completely move her legs and hands and c/o weakness, with the legs weaker as compared to the hands. Pt is having difficulty in
her self care activities, bed mobility, transfers and ambulation.

PMHx:
(+) Bronchial Asthma (since 7 y/o) with medication (Salbutamol and Comvibent for nebulization)
(-) Cardiac disease

- Pt is a non-smoker and non-alcoholic beverage drinker
- Pt is a senior high student
