Positive observations: N/A

1) One of the drivers did not wear full PPE at site. (Stopped him and instruct him to
wear full PPE before entering the site).
2) Observed there is one of the compactors was compaction in site and the
movement was not controlled by banksman. (Stopped the compactor movement and
bring the banksman, carried out a brief of requirements of the banksman and
Improvement opportunities: responsibility of both people. All movements shall be controlled by banksman if there
no banksman in front of the vehicle should be stop the vehicle till the banksman
came to guide).
