ANNOTATIONS: Amidst this time of pandemic, Modular Distance Learning is one of the modalities offered by
the school. Teacher takes the responsibility of monitoring the progress of the learners by the help of the parents.
That is why open communications between teacher and parents / guardians or teacher and learner is very
important being part of the norm. To ensure and monitor the progress of the learners at home, I constantly monitor
the learners performance through their modules submitted and recording their progress in the individual learning
monitoring plan. As a teacher, we should identify and record the need of each learner and be able to provide the
necessary interventions to address the needs. Attached above the sample of accomplished individual learning
monitoring plan of a learner with difficulty in particular learning area, the interventions provided and the current
status.

ANNOTATIONS:
I developed instructional material to simplify the topic and for the learners to
better understand the lessons since it is not well explained in their module. I
gave them additional performance tasks for the activity.

The problem is his parent/guardian couldn’t get his module in school because they
were busy working, so I conducted home visitations and deliver the modules right to
their house for him to continue answering it, asked him answer the modules and
scheduled date of pick up. I also conducted home visitations to the learners with
difficulty in coping time schedule in submission modules.

I developed lessons and learners activity sheets to supplement the modules that the
learners are using, I found out that some of my learners struggled in doing their task
specially in answering modules in a given time.So, I always gave them extra time and
extensions in submitting their outputs. I’d tried to be their agent and bridge by
motivating them to pursue their dreams thru education.

ANNOTATIONS:
In preparing lesson exemplars, I considered the learners level of ability. I make sure that I
checked their output before providing them new lessons. Lessons should be aligned with
the MELC. I also provide them activity sheets to supplement the modules.

Our colleagues is one of our support systems in this time of new normal. Having a GC,
non-stop meetings and collaboration with them helps me understand better all the
things that’s going on in everyday life of being a teacher. GC’s helps us to easily
disseminate needed information, raise some of our concerns and to have a quick
brainstorming to address the issues of our learners.

Taken during the module distribution & retrieval of modules in my class. Upon the distribution of
modules, the learning modules are to be distributed initially every Monday , in preparation for 1
week classes. Retrieval will be done next Friday when the distribution of the next set for week 2
will also be take place. Only the parents or guardians of learners are allowed to fetch and return
said modules while observing the necessary health protocols such as wearing facemask,
faceshield, use of hand sanitizer and observing physical distancing when doing so.
Parents/guardians upon entry to school should wear facemask and observe proper social distancing
of at least one-(1) meter apart while walking to and from the venue. These said routines will
continue until the end of every quarter of the school year.
