Subject: URGENT! Request for the issuance of SB RESOLUTIONS

Honorable Gentlemen and Mesdame:

Greetings of Peace and Sobriety!

This pertains to the abovementioned subject wherein the Office of the Municipal Mayor seeks and duly
requests the following documents:

1. A Resolution Amending Resolution No. 269. (Attached herewith is Resolution No. 269 marked as
Annex “1”).

May we implore before the August body to strike out highlighted words and rephrase the Title as well
as the Operative Clause of the said Resolution.

2. A Resolution Amending Resolution No. 228. (Attached herewith is Resolution No. 269 marked as
Annex “2”).

This is due to the Recommendations from the Department of Agriculture-Central Office.

Thus, may we implore before the August body for an Addendum in the said Resolution Approving the
Proposed Project and Allocation of LGU Counterpart. Herewith is the Proposed Project for the
KADIWA Grant for your reference. (Aherewith is the Project Proposal marked as Annex “3”).

Your positive and prompt response and consideration to the concern raised herein will be highly appreciated,
