Bước đầu tiên điều trị nhiễm khuẩn RHM: biện

pháp tại chỗ
• Dẫn lưu thoát mủ trong áp xe do răng: nhổ
răng, nội nha
• Rạch dẫn lưu mủ qua mô mềm

Không rạch thoát lưu sưng trong viêm mô tế bào tấy!

 CÂN NHẮC CHỈ ĐỊNH KS TRONG RHM
Tình huống lâm sàng chỉ định KS Tình huống lâm sàng không chỉ định KS

1. Sốt kéo dài trên 24h 1. Đau (→kháng viêm/giảm đau)

2. Triệu chứng toàn thân: mệt mỏi, chóng mặt, thở nhanh, 2. Sưng (→ kháng viêm)
hạch tại chỗ đau → nhiễm trùng nặng

3. Khít hàm → nhiễm trùng lan khoang quanh hàm, có thể 3. Nóng/đỏ (→ kháng viêm)
lan rộng gây nguy hiểm; khó thực hiện thủ thuật trong miệng

4. Dự phòng: bệnh nhân có vấn đề toàn thân: thấp tim, viêm 4. Chảy mủ → dẫn lưu mủ/nạo túi
nội tâm mạc nhiễm khuẩn, van tim/khớp nhân tạo

5. Bệnh lý miễn dịch kèm: AIDS, ung thư, bệnh tự miễn, suy 5. Áp xe khu trú (áp xe xương ổ, áp xe nha chu) →
giảm miễn dịch (giảm BCĐNTT chu kỳ, giảm 3 dòng tb máu, rạch thoát dẫn lưu
bệnh mạn tính không kiểm soát)

6. Sau cấy/ghép tạng (tim, thận, gan, tủy, implant trong 6. Dẫn lưu xoang (loại bỏ ổ nhiễm trùng → xoang
xương) tự lành hoặc PT sau đó)

• Penicillins bán tổng hợp (thêm -R
vào vòng acid 6-aminopenicilloic)
• Bị hủy bởi men β-lactamase
 β-Lactams
Aminopenicillins
• Phổ kháng khuẩn rộng.
• Mở rộng Gram (-): E. coli, Proteus, Salmonella,
Haemophilus, M. catarrhalis, Klebsiella, Neisseria,
Enterobacter, Bactoroides.
• Ampicillin, Ampicillin/sulbactam (Unasyn): PO/IV
• Amoxicillin (PO)
• Amoxicillin/clavulanate (Augmentin®)
• Sulbactam & clavulanic acid (do Streptomyces clavuligerus)
• Chống β-lactamase của VK
 β-Lactams
Phản ứng phụ
• 5% có phản ứng mẩn đỏ (penicilloic acid)
• Ampicillin gây rash trong 50-100% mononucleosis
• Quá mẫn (sốc phản vệ): 1/10,000
• Phát ban, phù mạch, hen suyễn, quá mẫn
• Tử vong: 10%
• Phản ứng da (-) vẫn có thể xảy ra quá mẫn
• Phải tránh tất cả thuốc β-lactams
 β-Lactams
PENICILLIN CHỐNG STAPHYLOCOCUS

• Methicillin, nafcillin, oxacillin, cloxacillin dicloxacillin.

• Chống lại hiện tượng thoái hoá do penicillinase.
• Hiệu quả trong điều trị S. aureus
• Không hiệu quả đối với các chủng Strep.
• Methicillin có độc tính thận
• Dicloxacillin: nồng độ trong huyết thanh cao nhất dưới
dạng uống
• Nafcillin: đường toàn thân
 β-Lactams
Penicillin chống Pseudomonas

• Ticarcillin, Piperacillin, Mezlocillin.

• Piperacillin/tazobactam (Zosyn; IV)
• Tazobactam (ức chế B-lactamase)
• Ticarcillin/clavulanate (Timentin; IV)
• Hữu hiệu đ/v Pseudomonas, E. coli,
Klebsiella, Enterobacter, Serratia & B. fragilis
• Ít hiệu quả đ/v vi khuẩn Gr (+)
• Thường phối hợp với aminoglycosides
 β-Lactams
CEPHALOSPORINS

• Beta-lactams bán tổng hợp có nguồn gốc từ những

chuỗi bên gắn kết thêm 7-aminocephalosporanic acid
• Đề kháng tốt hơn với β-lactamases
• Phản ứng phụ
• 5-10% phản ứng chéo ở BN dị ứng PNC
• 1-2% quá mẫn ở BN không tiền căn dị ứng PNC
• Phổ rộng hơn → nhiễm trùng cơ hội (candidiasis,
viêm đại tràng do C.difficile)
KHÁNG SINH ỨC CHẾ TỔNG HỢP PROTEIN
• Mục tiêu là ribosome của vi khuẩn
• 50S binders --- Macrolides, Clindamycin,
chloramphenicol, Streptogramins.
• 30S binders --- Aminoglycosides, Tetracyclines
 MACROLIDES
• Erythromycin (IV; Oral)
• Gram (+) : Staph. (MRSA kháng thuốc), Strep., Bordetella,
Treponema, Corynebacteria
• Không điển hình: Mycoplasma, Ureaplasma, Chlamydia
• Clarithromycin
• = Erythromycin
•  hoạt tính chống Gr(-) (H.I, Moraxella) & không điển hình
• Azithromycin
•  chống cầu trùng Gr(+)
•  chống H. influenzae & M. catarrhalis
 MACROLIDES
• Tác dụng phụ :
• 10-15% BN không uống đầy đủ erythromycin do
RLTH
• Vàng da
• Độc tai (liều cao)
• Tương tác thuốc :
• Oxy hoá do cytochrome p-450
• Gây tập trung nhiều hơn trong huyết thanh các
thuốc Theophylline, warfarin, astemizole,
carbemazepine, cyclosporine, digoxin, terfenadine
 Lincosamides
CLINDAMYCIN
• Clindamycin (IV; Oral), Lincosamides
• Phổ kháng khuẩn :
• Strep species, Staph (vài MRSA), B. fragilis,
anaerobes
• Dùng trong nhiễm trùng cổ sâu, viêm họng-
amidan mạn, áp-xe do R, phòng ngừa trứơc PT
• Sử dụng đồng thời với macrolides hay
chloramphenicol không cần thiết.
 Lincosamides
CLINDAMYCIN

• Tác dụng phụ:

• Viêm đại tràng giả mạc --- clindamycin >
cephalosporins > aminopenicillins
• Đau bụng, sốt, tiêu máu
• Tiêu chảy: ngày 4-9 của quá trình điều trị
• Đặc trưng: hồi phục sau 14 ngày ngưng thuốc
• Điều trị với Flagyl , Vancomycin
 AMINOGLYCOSIDES
• Neomycin, Gentamicin,Tobramycin, Amikacin
• Ức chế tổng hợp protein
• Phổ kháng khuẩn :
• Gram (-) : Pseudomonas, Proteus, Serratia, E. coli,
Klebsiella
• Neomycin
• S. aureus và Proteus
• Pseudomonas và Strep kháng thuốc
• Tác dụng phụ :
• Độc tai
• Độc thận
 ỨC CHẾ TỔNG HỢP DNA
FLUOROQUINOLONES

• Ciprofloxacin (IV/ Oral/Topical), Ofloxacin,

Levofloxacin (IV/ Oral)
• Ức chế DNA gyrase, tách DNA vĩnh viễn
• Phân bố rộng tận dịch mão tủy, nước bọt, xương,
sụn
• Hiệu quả chống gram +, gram -, Pseudomonas
• Ít hiệu quả đ/v yếm khí
 FLUOROQUINOLONES
• Tác dụng phụ:
• Nhức đầu, choáng váng , buồn nôn, lơ lửng
•  sử dụng trong thai kỳ, mẹ cho con bú, < 18 tuổi
• Tương tác thuốc: nồng độ theophylline, warfarin,
caffeine & cyclosporine
•  hấp thu khi sử dụng với cations
• Đau khớp = 1% ... Đứt gân cơ Achilles
OXAZOLIDINONE
Tác dụng: trên vi khuẩn Gram-dương.
Không tác dụng trên vi khuẩn Gram-âm.
Chỉ định lâm sàng: CĐ vi khuẩn Gram-dương kháng các KS khác
 METRONIDAZOLE

• Flagyl® (IV; Oral)

• Phân bố đến gần như tất cả các mô (dịch
não tủy, nước bọt, xương , áp xe ...)
• Phổ kháng khuẩn: yếm khí & KST
• Điều trị nhiễm Clostridium difficile & nhiễm
trùng yếm khí khác
• Tác dụng phụ: kích ứng đường tiêu hóa,
nhiễm độc hệ thần kinh
 KS THƯỜNG DÙNG TRONG NHA KHOA
KS sử dụng Nhóm Đặc tính quan trọng
Amoxicillin Diệt khuẩn - Dung nạp tốt đường uống
- Ít độc tính
- Đa số nhiễm trùng do răng còn đáp ứng
Amoxicillin + Diệt khuẩn Tương tự amoxicillin, thêm khả năng kháng β-lactamase
Clavunalte
Cephalosporin Diệt khuẩn CĐ trường hợp dị ứng PNC, tác động tốt trên đa số VK gây bệnh
trong miệng (không CĐ nếu dị ứng PNC týp I)
Metronidazol Diệt khuẩn Chống VK kị khí bắt buộc, không tác động VK kị khí tùy nghi
(Streptococcus); CĐ trong gđ hình thành áp xe
Clindamycin Kìm khuẩn CĐ: nhiễm trùng dị ứng PNC; phổ rộng, gồm VK kị khí; bội nhiễm
Tetracyclin Kìm khuẩn Đạt nồng độ trong xương và mô nướu; chế phẩm sử dụng tại chỗ
trong điều trị nha chu
Aminoglycosid Diệt khuẩn - dạng chích
/Kìm khuẩn - ĐT NKDR do VK gram (-)
- Kết hợp với thuốc khác trong NTDR nặng
- Độc tính trên tai và gan
Macrolid Kìm khuẩn Phát triển kháng thuốc nhanh đối với thuốc mới thuộc nhóm này
→ tránh sử dụng nếu có thuốc khác phù hợp
Fluoroquinilon Diệt khuẩn Moxifloxacin chống lại đa số VK gây NKDR; CĐ ở bn dị ứng PNC,
dùng thay thế nhưng không nên sử dụng thường xuyên

PLASMID
• Plasmid là những phân tử ADN nhỏ nằm trong bào tương, không
thuộc nhiễm sắc thể, có khả năng phân đôi độc lập gồm 2 thành
phần:
- Bộ gen xác định sự đề kháng với KS, mỗi gen trong bộ gen
đó kháng với 1 hoặc 2 loại KS
- Yếu tố truyền tính kháng thuốc RTF (Resistance Transfer Factor)

• Mỗi Plasmid có khả năng đề kháng nhiều loại KS cùng lúc.

• Plasmid có khả năng trao đổi giữa các vi khuẩn qua tiếp xúc với
nhau làm gia tăng tỉ lệ kháng rất nhanh trong nhiều chủng vi
trùng.
