I, TEOFISTO R, MANALO, of legal age, married, Filipino citizen and with residential
address at Brgy. Gulang-gulang, Lucena City, Philippines, after having been sworn to in
accordance with law, hereby depose and state;
That my late father, Guillermo Manalo, was the registered owner ½ portion of a parcel of
land, including the improvements thereon, located at Brgy. Sampaloc I, Sariaya, Quezon which
is more particularly described as follows:
A parcel of land (Lot No. 3022 of the Cadastral Survey of Sariaya), with the
improvements thereon, situated in the Municipality of Sariaya. Bounded on the E. by
a creek; on the SE. by Lot Nos. 3019, 3020 and 3021; on the SW. by Lot No. 3015;
and on the NW. by Lot No. 3014. Beginning at a point marked “1” on plan, xxxxx
xxxxx xxxx; containing an area of FIFTEEN THOUSAND THREE HUNDRED AND
ONE SQUARE METERS (15,391 SQM), more or less. All points referred to are
indicated on the plan; bearing true; declination 0 deg. 10’ to deg. 50’ E.; date of
survey, January 1917 to January 1921.
That the Original Owner’s Copy of the above-described Transfer Certificate of Title
pertaining to my deceased father, Guillermo Manalo, was originally in my possession and
custody since 1977;
That on June 2021, my first cousin, Marieta Manalo-Valdoria, through his son in law,
Michael Jumawan, barrowed from me the said Transfer Certificate of Title and promised me that
they will return the same. However, until today, despite repeated demands, they still refused to
return the said Title and as a matter of fact, they are no longer communicating with me and I am
afraid that they may have already convey the property embraced therein in favor of a third
person;

Quezon on the date and place above stated. Affiant personally appeared and exhibited to me his
government-issued identification card with ID No. SC 2016-09-30-23257 as competent proof of
his identity.
