On Monday, 28 November 2022 at Jetty-2 L49, around


10.25 LT, the Meindo team was conducting shifting
support of cold shoe, by putting a wooden block on it
and hitting the block by 10kg hammer. The wooden
block was held by a worker (IP) while his colleague hit it
with hammer. IP accidentally placed his left ring finger
between wooden block and steel support during the
impact by hammer. As result IP sustained left ring finger
injury.

The IP has been sent to site clinic for further treatment.

On 25 Nov 2022, around 09:25LT, one of the SBC PH


found that the ICC tag (ICC-10521) was lying on the
working platform on his work area.

The ICC tag found by PH which should be still in place


(as bleed point) to indicate that spool must be kept
disconnected since it used for blowing point but has
been removed.

The ICC remains open.

located in HRSG Steam Drum in PR.

The line having ICC#10501 on 23 Nov but on 24 Nov


2022, found out that Tee-joint had been reinstated.

A-212 29 NOV 2022 NEAR MISS BHI A (-) ISSUED

On Tuesday 29 November 2022, a BHI Technician had


an instruction to move the six-way manifold from PR
Grating level to MR Grating level. While moving the
manifold, two handles on the manifold (5" length, 65gr
weight each) fell and passed through the grating, landed
on the ground, 8m below.
The drop zone was accessible by pedestrians. No
personnel injured in this incident.

Noise Personal Protection Zone: ✓ MANDATORY TO HAVE and BRING HEARING
1. LP Steam Blow: Double hearing protection: < 10 m, Single hearing protection: 10 – 40 m PROTECTION DEVICE (EARPLUG / EARMUFF)
2. HP Steam Blow (with silencer): Double hearing protection: < 20 m, Single hearing protection: 20 – 100 m

over to MPO (SH-1 signed).

Recommendation
