METHOD STATEMENT SHUTTERING AND FORM WORKS

1. Introduction and Scope


This method statement provides the controlled sequences of work, methodology and safety precautions
that will be used to complete the Shuttering and Form works. The content of this method statement
reflects the findings of the relevant risk assessment.

2. Location of Activity
Inside/outside of the building.

3. Scope of Work.
The method statement covers all activities related to the Shuttering and Form works.

General Description

The purpose of this Method Statement is to give a general outline of the methodology that will be used
for Shuttering and Form works at, The WTC to identify the risk associated during the Shuttering and
Form works and to identify the quality control system to ensure full compliance with the client’s
requirements and as per the project specification.

The works shall be executed in accordance with the following methodology and sequence;

1. Preparation of Shuttering and Formwork

The forms shall be smooth, mortar tight, true to the required lines and grade and of
sufficient strength to resist springing out of shape during the placing of the concrete.
Dimensional tolerances shall be according to the ACI 347-78 “Recommended Practice for
Concrete Formwork”. Commonly formwork material shall be used 18 mm waterproof film faced
plywood with wooden bracing or metal formwork can be used upon the requirement of structure
unless otherwise recommended by the EMPLOYER REPRESENTATIVE.

Temporary openings shall be provided in the formwork to facilitate this work. The inside
surfaces of the formwork shall, immediately prior to final erection, be coated with mould oil. The
mould oil shall be of approved type and shall be applied uniformly and the quantities used shall
be the minimum consistent with its purposes. The Contractor shall ensure that all steel
reinforcement and adjoining concrete surfaces are kept free of mould oil.

All concrete faces shall be left as struck with a fair face, true to line finish. After careful
inspection, all superfluous fines and similar projections shall be carefully removed. No render or
other applied finish shall be used to obtain a fair face to the concrete. All concrete faces to be
exposed in the finished works shall be adequately protected against damage and surface staining
during the execution of the works.

All exposed sharp edges shall be chamfered with triangular fillets not less than 50 mm by
50 mm. Forms clamps or bolts shall be used to fasten forms. The use of ties consisting of twisted
wire loops to hold forms in position during the placing of concrete will not be permitted.

Immediately before the concrete is deposited, the formwork shall be thoroughly cleaned
out and freed from sawdust, shaving, wire cuttings, dust, sand, clay and all other deleterious and
extraneous materials.

They shall be such type that shall be entirely removed or cutback 40 mm or more below
the finished surface of concrete. No metal part shall be retained after remove the formwork.

Form work shall be complete and inspected by the EMPLOYER REPRESENTATIVE


before starting concrete activity.

2. Removal of Shuttering and Formwork

The EMPLOYER REPRESETATIVE shall be informed in advance to strike any


formwork. At the time of striking any formwork, the concrete shall be of sufficient age and
strength for it to withstand the effects of striking, including the effect of thermal shock or the loss
of surface durability upon premature exposure, and to be able to withstand the resulting stresses
without adverse effects

The time at which the formwork is strike shall be strictly followed as specified in DM
specification Section 3, clause 3.13.7 and General specification volume II section 4.32.5.

No formwork is to be removed, until approval received from the EMPLOYER


REPRESENTATIVE, No supports for the shuttering shall be left in finished concrete without
EMPLOYER REPRESENTATIVE Permission. During striking the form extremely care shall be
take not to damage the concrete surfaces

Where internal metal ties are permitted, they or their removable pairs shall be extracted
without damage to the concrete and the remaining holes filled with mortar and match with the
external surface of concrete.

3. Inspection and Record

The clearance shall be received from the site engineer after completing the
Shuttering work. QC Inspector shall be inspected and witness in all cases acceptance in the
particular activity shall be confirmed. After that the RFA shall be provided for requesting
approval shuttering work by the EMPLOYER’S REPRESENTATIVE in sufficient time to allow
an inspection to be made. Any concrete pour shall not be allowed until the approval received
from EMPLOYER’S REPRESENTATIVE.

4. Responsibility

 Project Engineer:

The Project Manager and the Construction Manager have the responsibility for the overall operations of
this statement. The Project Manager and Construction Manager have the responsibility to implement the
Method statement and to delegate the responsibility to a Site Engineer and the QC Engineer to directly
supervise the work on site. The site Engineer is responsible for ensuring that all the works are executed
in accordance with the specified requirements.
The Project Manager will be responsible for the following.

 Plan and schedule the jobs, determine priorities, control workload, inspect projects and keep sufficient time
records for workflow and outputs.
 Performs technical responsibilities and make decisions to a substantial extent.
 Reviews and interprets proposed designs, drawings, and specifications for appropriateness to required
function and/or standard and initiates revisions.
 Ensure that project operations are in compliance with design specifications
 To provide all the necessary information to the site team.
 Engineer/Supervisor in charge to conduct a toolbox meeting and communicate both the Method Statement
& Risk Assessment to the workers before commencement of the activity.
 All safety precautions shall be followed as per established project safety procedure.
 Only experienced and skilled labour shall be engaged for carrying out screed work.
 People involved in the construction work shall wear PPE such as safety helmets, safety shoes, safety harness,
gloves safety glass, mask etc.as required.
 All Hose pipe joints are properly connected and tied with clips.
 Safety officer shall check and ensure that all safety precautions are followed.
 All the safety equipment’s shall be periodically inspected and checked for its durability before using them to
labor.
 Manual handling procedures to be followed
 Any incident or near miss incident shall be reported immediately to the concerned safety officer.

Site Supervisor:

The Site supervisor will be responsible for the following

 Distribution of the responsibilities to his site team.


 To provide all necessary information to his site team.
 To ensure that the works are carried out in accordance with the WMS and RA.
 To ensure the MS, ITP, & RA is communicated to the Forman and the site working team on a regular basis to
ensure that the proper work method is followed.
 To check and verify that the works completed complies with the specification prior to requesting inspection
by the consultant
 To ensure that inspections are carried out by the consultant through the main contractor’s QA/QC Engineer
prior to the next phase of activities
 To review and discuss any request that might be raised by the site working team.
 Primarily ensure that the job is being carried out to the approved WMS and ITP.

Foreman:

The Forman supervisor will be responsible for the following

 To provide the Site supervisor with daily progress reports and discuss with him planned works for the next
day.
 To ensure that all safety measures are maintained at site and workers have proper training for following all
current safety guidelines and housekeeping in accordance with the Main Contractors Safety Officer.
 To ensure good housekeeping is practiced and proper tools are being used by the site team.
 To ensure that all persons involved in the works are aware of their duties, and that they have enough
understanding of safety procedures.
 To conduct frequent toolbox meeting on safety awareness and to maintain a log of all meetings for
inspection.
 To keep a record of all toolbox talks.
 Liaises with the required personnel at the site for the required inspections with regards to safety.
 Attends all the safety coordination meetings.
 Ensure that the basic and activity specific PPE is worn regularly by the site working team.

5. Equipment Requirements

The requirements that will be engaged for Shuttering and Formwork will be as follows:
 Site In-charge.
 Charge Hand.
 Skilled Labour.
 Unskilled Labour

6. Health and Safety


Strictly followed as per the manufacturer’s Health and Safety recommendations for handling and use of the
materials. Ensure all involved personnel shall be aware of the same.
