1.0 Purpose: Safe Work Procedure & Permit to work system is to provide a clear understanding of the steps
required, hazards involved and precautionary measures to be taken to perform the job safely.
2.0 Reference: Nil
3.0 Objectives: To provide a standardized & uniform procedure for obtaining authorization to perform work.
o Requiring special consideration to ensure the safety of personnel, and protection of facilities.
o To provide a system to perform a job safely and a Clear understanding between the working
agencies, executing agency and ownership of the equipments / areas.
4.0 Applicability: Inside Tata Steel Works
5.0 Responsibility: Working department, Contractors and Consultants
6.0 Procedure:
Sl. No.
1) The owner department will isolate the equipment to ensure “It is Safe” to work prior to issue of any
work permit. The isolation will be as per written procedure which is attached to the work permit. A
written isolation procedure is required for every job (Which requires isolation).

2) Supervisor (Permit requested by) of the group performing the work will make the necessary contact
with the owner supervisor (Permit Issued by) concerning the job. Supervisor (Permit taken by) of
the group who will carry out the work will be responsible for “Permit taken by”. If the job is carried
out by the Contractor under Central Agency, Central agency will countersign, and will be
responsible for Safe execution of the job.

3) Prior to start the work, the work permit will be filled up in duplicate to clarify the type of work
involved, precautions taken, etc. Supervisor of the group (permit requested by) performing the job
will be responsible for filling out the form. Owner supervisor (Permit Issued by) will be responsible
for explaining the proximity hazards of the area and necessary precautions. Supervisor (Permit
taken by) of the group doing the job will be responsible for carry out the job safely including
proximity hazards, PPEs of their employees, proper tools & tackles.

4) Before the permit is issued, the issuer will ensure that all precautions in co-ordination with other
agencies/ running equipments / other activities are taken into consideration. Before the permit is
issued, executing agency will ensure that all precautions indicated in the permit format are being
considered. The original copy of the signed permit is then given to the supervisor of the group
responsible for doing the job.

Guidelines:

1) Safe Work Procedure should be made in advance, in the prescribed format EHSMSM / WORKS /
446 / 4009. It should contain hazards involved and precautions of the job to be performed including
proximity hazards.
2) It should contain safety measures as indicated in safety standards, local rules & procedure (By dept.)
and statutory requirements. Contractor working under Central agency - SWP should be made by
Central agency in consultation with departmental agency.

3) Training of Safe Work Procedure should be provided to all Contractor’s Supervisor by department
regarding hazard involved during the job and its precautions.

4) Associated hazards and necessary precaution of the job being carried out, will be imparted to all
contractor workers by Contractor’s Supervisor. Department should ensure that permit should be
given only after verifying the Contract workers are well aware of Safe Work Procedure.

5) Job should be carried out after getting Permit to Work System. It should be filled up after
anticipating the proximity hazard. Self certification by contractor should be taken for all tools &
PPEs etc.

6) Permit to work format is valid for only one area.

7) For confined space work: To work in confined space “permit to work” format should be issued
against departmental confined space permit number (EHSMSM / WORKS / 446 / 4002).
Departmental confined space number should be generated against Confined space clearance
(EHSMSM / WORKS / 446 / 4002) taken from Competent person.

The list of competent persons for Confined space:


A person declared by Chief Inspector of Factories as competent under Section 36 of Factory Act 1948
based on qualification, experience and facilities available at his disposal to certify a confined space as
safe to work based on test / examination of atmosphere carried out by himself,
The list of competent persons are available in Intranet, safety site.

8) Permit to work Format:

Permit requested by – Permit will be requested by the departmental agency under whom job
will be carried out.

Permit Issued by: Permit will be issued by Owner of the area / equipment.

Permit taken by: Permit will be taken by the agency that will perform the job.

9) Permit to be signed by not less than Supervisor / Shift In-charge / Dept. authorized person

10) Audit should be conducted before and during the job to evaluate the severity level.

11) Clearance for gas hazardous area shall be taken in Form# EHSMSM/WORKS/446/4001 by the
executing agency and to be issued by the owner.

12) Electrical Power cutting clearance will be given in Form# EHSMSM/WORKS/446/4005 and for
Electrical job which requires grounding / discharges, clearance will be given in Form#
EHSMSM/WORKS/446/4010. The Power cutting clearance will be requested by executing agency.

13) Permit requested means - Maintenance group/central agency after verifying safe condition for
working at height & gas cutting /working agency,
Permit issued means - Departmental operation / maintenance group,
Permit taken means - Working agency / contractors.
Owner Department means – The department who has got ultimate operational control on
equipment / process and activities, also in his capacity issue the permit to executing agency.

14) Confined Space work clearance will be given in separate in Form # EHSMSM / WORKS / 446 /
4002 (Part 1 & Part 2). For Hot Job (Gas cutting / welding/burning/open flame/use of power
tools/grinding) in Confined space, the test of atmosphere shall be carried out for O2, toxic gases,
flammable gases (No hot work shall be permitted unless the Explosive meter reading is zero) by
competent persons before issue of this work permit and shall be repeated if there is discontinuity of
hot job for more than two hours. The findings may be mentioned in space for special instructions at
Sl.No.7 of front page in Form # EHSMSM / WORKS / 446 / 4007.
First part of Confined Space form (Form # EHSMSM / WORKS / 446 / 4002) is to be filled by competent
person. Against above form departmental authorized person will fill up second part of the form (Form #
EHSMSM / WORKS / 446 / 4002) for every job (permit to work). The condition of confined space is to be
monitored as specified by competent person.

Concerned department shall maintain the following records:


• A register of all Controlled/Confined Spaces within their plant ownership.
• A list of people who are authorised to approve confined space check sheets.
• A list of people who can assess the competency of those who perform confined space work within
the department.
• A list of accredited CSRP an Service Providers who are accredited to perform confined space work
within the department.

• Shall maintain record on entry work permit issued to working agency, for at least one year.

• Record for Incident/Accident is to be maintained at the department and is also made available at
Safety Department for at least five years.

Note: Emergency Rescue system should be made available before starting in any confined space work.

15) Clearance for working at height will be given only when the persons (dept. agency under whom job
will be carried out) giving clearances ensures availability of all safety check points ( AS
APPLICABLE) indicated below :
• Safe approach / Access Ladder / Rest platform if needed.
• Provision for extended platform if required
• Vertical clearance of at least 2 meter OR Supervision
• Safe Scaffolding including physical conditions
• Conditions of hand rail / middle rail & toe guard

• Fencing of down area / ground area


• Experienced & / or Trained persons (preferably certified riggers), contractors to provide evidence –
details of experience / training undertaken / certification done on competency will be mentioned by
the concerned working agency in a register.).

• Use of Full Body Harness with arrangement of life line support
• Check for fitness of persons to work at height
• Clearance for Over head line Vertical>3.7 m & horizontal >1.2m

16) Clearance for gas cutting welding will be given only when the persons (dept. agency under whom
job will be carried out) giving clearances ensures availability of all safety check points (AS
APPLICABLE) indicated below :
• Physical condition of gas cylinders of O2 / DA / LPG
• Vertical Position of cylinders
• DA Cylinders should not be kept in sun light
• Clamping of hoses with jubilee clamp
• NM Gas Cutting equipment with flash back arrestor
• Use of hand gloves, helmet, shoes, Face shield
• Availability of water bucket / Fire Extinguishers / Fire Hydrants / sand as per need.
• Condition of Electrical cables, welding cable, cable joints
• Proper system & equipment grounding (return connection to the equipment / job being welded).
• Condition of regulators /pressure gauge / welding holder
• Removal of any source of flammable material /chemicals, oil / solvent, grease or gases.
• Experienced & / or Trained Persons ( preferably certified welder / gas cutter , contractors to provide
evidence)
STRIKE OUT WHICH IS NOT APPLICABLE

Note: However Clearance for gas cutting / welding is not required if it is being carried out in permanent
fabrication yard / shed or welding shop but safe conditions must be checked periodically.
If the job is being done under central working agency like MSG (Mech) / MED (Mech) / Engg.
Services etc, clearance for gas cutting /welding shall be verified & signed by them only.

17) All operations of equipment/source/switches shall be done by department (Contact worker should
not be allowed).

18) BEWARE OF OVERHEAD MOVING CRANES & RUNNING CONVEYORS

19) Clearance for Hydraulic System should be given after putting off hydraulic system, isolating the
valve, pushing the emergency switch, depressurizing the system & draining the accumulators as per

20) After General Shift, executing supervisor will have to contact shift supervisor for carrying out the
job.

21) Once the permit is issued, working agency will be responsible for compliance to all safety
precautions and use of all specific personal protective appliances and Tools & Tackles.

22) During cleaning, Use Dust Mask & Do Not Touch Any Running Equipment. Do not throw swiped
material from height.

23) During Painting Job, Use Hand Gloves & Do Not allow spillage. Confirm to working at height
requirement as applicable.

24) Communicate to all concern whose activity may get affected by the ongoing work. Barricade the
area through display & putting red tape as applicable.

25) The list of persons working may alternatively be maintained in a register. ATTACH EXTRA
SHEET IF REQUIRED.

Central Working Agency: MSG (Mech) / MED (Mech) / Engg. Services / MS (Electrical) / MED
(Electrical) / Equipment Maintenance / Automation & Telecommunication etc.

Note: The work permit must state the specific work area / scope of work which is permitted. No work
outside this scope/area is permitted unless a new work permit (Isolation procedure) has been issued.
The job must be monitored (periodically) by the Owner / Executing agency for compliance to the
work permit. Field audit as per DuPont methodology has to be carried out during execution of the
job. Any non-compliance should be rectified. If any major non-compliance occurs (e.g. – Incorrect
Scaffold, working outside scope etc.) the job must be stopped immediately and the same should be
rectified immediately. All major non-compliance should be reported to concerned head.
