1. Hormone glucocorticoid và trục HPA

TIÊN ĐOÁN ĐỘC TÍNH CỦA GC – CÁCH PHÒNG NGỪA
• Liều cao trong thời gian ngắn (< 1 tuần) kiểm soát được bệnh và ít độc tính hơn
chế độ liều thấp nhưng thời gian dài ( prednisone 5 mg, > 3 tháng)
• Phòng ngừa suy vỏ thượng thận: dùng thuốc liều duy nhất không chia liều nhỏ, 8–
9 h sáng, hàng ngày hoặc cách ngày, chọn loại có thời gian tác động ngắn/ trung
bình
• Thời gian dùng thuốc > 2 tuần, dù với liều sinh lý, phải ↓ liều từ từ trước khi ngưng
thuốc, bắt buộc theo dõi bệnh nhân 1 năm sau khi ngưng thuốc để kịp thời bổ sung
corticoid khi có stress
• Nếu giảm liều quá nhanh có thể làm trầm trọng thêm bệnh, nếu giảm liều quá
chậm sẽ ↑ các tai biến do GC

trầm trọng thêm bệnh qua trung gian miễn dịch

Suy vỏ thượng thận
• Chia nhỏ liều gây suy vỏ thượng thận > liều duy nhất
• Tối > sáng
• Dù dung liều thấp nhưng kéo dài nhiều tháng vẫn phải giảm liều trước khi ngưng điều trị. (>2-3 tuần)
• Sử dụng thuốc cách ngày
Phù hợp với bệnh nhân lupus ban đó, hen suyễn, viêm màng mạch, không phù hợp với chống thải ghép , rối
loạn đông máu, bệnh ác tính
Giảm liều chậm (vài tháng) và theo dõi bn chặc chẽ đến khi đạt được liều có hiệu lực thấp nhất thì chuyển
sang chế độ cách ngày
• Theo dõi chức năng thận, thử nghiệm đánh giá chức năng thận (cosyntropin, hạ đường huyết bằng insulin,
metyrapon CRH và ACTH liều thấp)
• Trong 6 tháng đầu ngừng corticoid sau một đợt điều trị dài ( 2-3 tuần) với liều dược lý prednisone > 7,5 mg/ngày
nếu có stress thì phải bổ sung GC:
Stress nhẹ : hydrocortisone 100 mg x 2-3 lần/ngày
Stress nặng: hydrocortisone 400-500 mg/ngày
Sau khi qua khỏi cơn cấp dùng liều sinh lý thêm 4 tuần

- Lympho T, B, đại thực bào, tb nội mô mạch máu màng hoạt dịch… ⇒ các tự kháng thể
(anti CCP, RF…) và các cytokines (TNFα, IL6, IL1…), collagenase, stromelysin, elastase
Tác động gây hại:
- Phản ứng viêm ngày càng gia tăng ⇒ tập hợp dày đặc của các TB MD và xâm nhập vào
màng hoạt dịch nằm trong khoang khớp⇒ nhiều cytokine + yếu tố tăng trưởng⇒ ↑ phù nề,
tân sinh mạch máu và tăng sinh màng hoạt dịch. Bao hoạt dịch phì đại xâm lấn vào các mô
xung quanh. Mô hạt hình thành, bao phủ sụn khớp và dẫn đến sự hình thành pannus.
- Pannus là mô mạch máu bao gồm các tế bào lympho, đại thực bào, mô bào, nguyên bào
sợi và tế bào mast. Pannus có thể ăn mòn và phá hủy sụn khớp, dẫn đến bào mòn
xương⇒ viêm, co rút, đứt gân, lỏng lẻo dây chằng, lệch khớp và biến dạng khớp.

Tiêu chuẩn của Hội Thấp khớp học Hoa Kỳ (ACR) 1987
- Hiện nay tiêu chuẩn này vẫn đang được áp dụng rộng rãi nhất trên thế giới và Việt Nam đối với thể biểu hiện nhiều khớp và thời gian
diễn biến viêm khớp trên 6 tuần.
+ Cứng khớp buổi sáng kéo dài trên 1 giờ.
+ Viêm tối thiểu ba nhóm khớp: sưng phần mềm hay tràn dịch tối thiểu 3 trong số 14 nhóm khớp sau (kể cả hai bên): khớp ngón gần
bàn tay, khớp bàn ngón tay, khớp cổ tay, khớp khuỷu, khớp gối, khớp cổ chân, khớp bàn ngón chân.

Chẩn đoán xác định: khi có > 4 tiêu chuẩn. Triệu chứng viêm khớp (tiêu chuẩn 1- 4) cần có thời gian diễn biến ≥ 6 tuần và được xác
định bởi thầy thuốc.
Tiêu chuẩn ACR 1987 có độ nhạy 91-94% và độ đặc hiệu 89% ở những bệnh nhân VKDT đã tiến triển. Ở giai đoạn bệnh mới khởi
phát, độ nhạy chỉ dao động từ 40-90% và độ đặc hiệu từ 50-90%.

TREATMENT
Goals of therapy
Patient education, alleviation of pain and swelling, prevention of structural damage, and
preservation of function
Drug:
o Initial therapy with antiinflammatory medications (NSAIDs, corticosteroids)
o Disease-modifying antirheumatic drugs - DMADs:
• Conventional DMARDs: hydroxychloroquine, sulfasalazine, methotrexate, leflunomide
• Biologic DMARDs: TNF inhibitors (etanercept, adalimumab, infliximab, golimumab,
certolizumab pegol), T cell costimulatory inhibitor (abatacept), IL-6 receptor inhibitors
(tocilizumab, sarilumab), anti-CD20 antibody (rituximab)
• Targeted synthetic DMARDs: JAK inhibitors (tofacitinib, baricitinib, upadacitinib)

GC:
• Lợi ích của liệu pháp GC phải được cân bằng với những nguy cơ tiềm ẩn
• Tuân theo các khuyến nghị hướng dẫn liên quan đến liều lượng liệu pháp

• Agreed that the risk of harm is low for the majority of patients at long-term dosages of 5 mg
prednisone equivalent per day, whereas at dosages of >10 mg/day the risk of harm is elevated.
• At dosages between >5 and 10 mg/day, patient-specific characteristics determine the risk of harm.
• This means general and glucocorticoid-associated risk factors and protective factors such as a
healthy lifestyle should be taken into account when evaluating the actual and future risk.

Initiation of treatment in DMARD-naive patients with moderate-to-high disease activity
• Initiation of a csDMARD without short-term (<3 months) glucocorticoids is conditionally
recommended over initiation of a csDMARD with short term glucocorticoids.
• Initiation of a csDMARD without longer-term (≥3 months) glucocorticoids is strongly recommended
over initiation of a csDMARD with longer-term glucocorticoids.
who are not at target with DMARD
• Addition of/switching to DMARDs is conditionally recommended over continuation of glucocorticoids
for patients taking glucocorticoids to remain at target.
• Addition of/switching to DMARDs (with or without IA glucocorticoids) is conditionally recommended
over the use of IA glucocorticoids alone for patients taking DMARDs who are not at target.
