I. Vàng da sơ sinh
Yếu tố nguy cơ vàng da
Almost all newborn infants develop a total serum or plasma bilirubin (TB) level greater than 1
mg/dL (17 micromol/L), which is the upper limit of normal for adults
●In term newborn infants, bilirubin production is two to three times higher than in adults. This
occurs because newborns have more red blood cells (hematocrit between 50 to 60 percent) and
fetal red blood cells have a shorter life span (approximately 85 days) than those in adults. The
increased turnover of more red blood cells produces more bilirubin.
●Bilirubin clearance is decreased in newborns, mainly due to the deficiency of the enzyme
uridine diphosphogluconurate glucuronosyltransferase (UGT1A1). UGT activity in term infants
at seven days of age is approximately 1 percent of that of the adult liver and does not reach adult
levels until 14 weeks of age [3,4].

Thiếu men: Erythrocyte enzymatic defects (eg, glucose-6-
phosphate dehydrogenase [G6PD] deficiency [17], pyruvate
kinase deficiency, and congenital erythropoietic porphyria)
Khiếm huyết màng HC: Inherited red blood cell membrane
defects (eg, hereditary spherocytosis and elliptocytosis).
Miễn dịch: Isoimmune-mediated hemolysis (eg, ABO or Rh(D)
incompatibility)
Nhiễm trùng
Phá hủy: Other causes of increased bilirubin production due to
increased red blood cell breakdown include polycythemia or
sequestration of blood within a closed space, which occurs in
cephalohematoma. Macrosomic infants of diabetic mothers
(IDM) also have increased bilirubin production due to either
polycythemia or ineffective erythropoiesis.

Tăng chu trình gan ruột: The major causes of increased enterohepatic
circulation of bilirubin are breastfeeding failure jaundice, breast milk
jaundice, or impaired intestinal motility caused by functional or
anatomic obstruction.
Breast milk jaundice:
o It typically presents after the first three to five days of
life, peaking within two weeks after birth, and
progressively declined to normal levels over 3 to 12
weeks
o Infants commonly have TB levels >5 mg/dL (86
micromol/L) for several weeks after delivery
o Hyperbilirubinemia is generally mild and may not
require intervention, it should be monitored to ensure
that it remains unconjugated and does not increase
o If TB levels begin to increase or there is a significant
component of conjugated bilirubin, evaluation for
other causes of hyperbilirubinemia should be performed
including neonatal cholestasis
o If after evaluation, breast milk intake is the only
remaining viable factor, breastfeeding can continue
with the expectation of resolution by 12 weeks of age
and that the hyperbilirubinemia is in the safe zone

Điều trị
o Quang trị liệu:
Cơ chế: chuyển bil tự do sang bil tan trong nước và thải ra ngoài. 3 cơ
chế:
Structural isomerization to lumirubin – Phototherapy converts
bilirubin into lumirubin via structural isomerization that is not
reversible [19]. Lumirubin, a more soluble substance than
bilirubin, is excreted without conjugation into bile and urine.
This is probably the principal mechanism by which
phototherapy reduces the TB concentration.
Photoisomerization to a less toxic bilirubin isomer –
Phototherapy converts the stable 4Z,15Z bilirubin isomer to
the 4Z,15E isomer, which is more polar and less toxic than the
4Z,15Z form. Like lumirubin, the 4Z,15E isomer is excreted
into bile without conjugation. Unlike structural isomerization
to lumirubin, photoisomerization is reversible; however,
clearance of the 4Z,15E isomer is very slow and the
photoisomerization is reversible. Thus, some of the 4Z,15E
isomer in bile is converted back to the stable 4Z,15Z isomer. As
a result, this pathway may have little effect on TB values. In
addition, standard laboratory measurements do not distinguish
between the isomers, so these measurements do not reflect
these changes. Nevertheless, photoisomerization does reduce
the amount of potentially toxic bilirubin by rapidly converting
15 percent to a non-toxic form.
Photo-oxidation to polar molecules – Photo-oxidation
reactions convert bilirubin to colorless, polar compounds that
are excreted primarily in the urine. This is a slow process and
accounts for a small proportion of bilirubin elimination.

theo liều chiếu cho đến 55
For TB levels ≥20 mg/dL (342 micromol/L), phototherapy should be administered continuously,
until the TB falls below 20 mg/dL (342 micromol/L). Once this occurs, phototherapy can be
interrupted for feeding and parental visits.

Ngừng chiếu khi Bilirubin dưới ngưỡng chiếu 25-50 umol/L (1.5-3
mg/dL)

Tác dụng phụ: HC da đồng, đỏ da, mất nước, tăng thân nhiệt, hủy
DNA, giảm Riboflavin
Chống chỉ định:
HC mật đặc
Bilirubin trực tiếp > 20%
o Thay máu:
Chỉ định trong 1 số trường hợp
Bilirubin tăng >20 mg/dL và vẫn tăng sau khi chiếu đèn 24h
Bilirubin tăng >1mg/dL/giờ dù đã chiếu đèn và Hb 11-13g%
Bilirubin máu cuống rốn >4.5 mg/dL và Hb <11g%
The dashed lines for the first 24 hours indicate uncertainty due to a wide range of clinical
circumstances and a range of responses to phototherapy.
Immediate exchange transfusion is recommended if the infant shows signs of acute bilirubin
encephalopathy (hypertonia, arching, retrocollis, opisthotonos, fever, high-pitched cry) or if TB
is ≥5 mg/dL (85 micromol/L) above these lines.
Risk factors include isoimmune hemolytic disease, G6PD deficiency, asphyxia, significant
lethargy, temperature instability, sepsis, and acidosis.
Note that these suggested levels represent a consensus of most of the committee but are based on
limited evidence, and the levels shown are approximations. During birth hospitalization,
exchange transfusion is recommended if the TB rises to these levels despite intensive
phototherapy.
For readmitted infants, if the TB level is above the exchange level, repeat TB measurement every
two to three hours and consider exchange if the TB remains above the levels indicated after
intensive phototherapy for six hours.
II. Vấn đề thường gặp ở sản phụ trong những ngày đầu hậu sản
1. Chăm sóc thường quy
Tốc độ co hồi TC:
o Bình thường sẽ co hồi 1cm/ngày
o Quá trình co hồi:
Sau sinh TC co cứng liên tục 3-4h sau đó có những cơn co bóp để tống
sản dịch ra ngoài
Ngày đầu hậu sản bề cao TC 13-14cm
Ngày 12-13 TC không còn sờ được trên bụng nữa
Yếu tố giúp co hồi nhanh hơn:
Con so
Bú sớm
Yếu tố làm chậm co hồi:
Nhiễm trùng
Sản dịch: là hỗn hợp mảnh vụn màng rụng, cục máu đông nhỏ và dịch tiết từ vết
thương đường sanh
o Trong 2-3 ngày đầu: sản dịch đỏ tươi sậm
o Ngày 4-8: loãng hơn, lẫn chất nhầy như máu cá
o Ngày 9-12: chất nhầy trong, ít đi dần
o Ngày 12-18: có thể chút máu tươi trong 1-2 ngày, đó là kinh non
o Yếu tố giúp tránh ứ đọng sản dịch:
Vận động
Cho bú sớm
Thay băng vệ sinh đồng thời vệ sinh thường xuyên
Sự lên sữa
2. Vấn đề đường niệu:
Bí tiểu sau sanh:
o Định nghĩa: sau sinh 6h or sau rút ống thông tiểu 6h or khi nước tiểu tồn lưu
>150ml mà không đi tiểu được
o Nguyên nhân:
Chén ép bàng quang quá mức làm liệt bàng quang
Thay đổi cấu trúc giải phẫu
Phù nề đường tiểu
Tổn thương tầng sinh môn
o Yếu tố nguy cơ:
Sinh con to
Chuyển dạ kéo dài
Sinh giúp bằng dụng cụ
Chấn thương âm hộ, tầng sinh môn
Mổ lấy thai khi chưa vào chuyển dạ
Gây tê màng cứng
o Điều trị:
Thông tiểu
Kết hợp:
Tập đi tiểu
Kháng sinh
Kháng viêm
o Dự phòng:
Cho sản phụ đi tiểu trong quá trình chuyển dạ, nếu không đi được thì
thông tiểu mỗi 4h, sau 2 lần vẫn chưa sanh nên thông tiểu lưu
Nên lưu thông tiểu 6h khi có các yếu tố nguy cơ trên
Nên lưu thông tiểu 12h sau sinh mổ
Khuyến khích đi tiểu sớm và vận động, không nín tiểu
Tiểu không tự chủ
Dò bàng quang-âm đạo
3. Vấn đề vết thương âm hộ-tầng sinh môn:;
Tụ máu: đau, căng tức vùng vết may, thường có cảm giác mắc rặn, sở thấy 1 khối đau
thốn
Nhiễm trùng vết may tầng sinh môn
III. Tránh thai thời kỳ hậu sản
3 đặc trưng trong thời kì hậu sản
o Tăng đông
o Tiết sữa với tăng prolactin
o Buồng trứng phóng noãn không ổn định
Các vấn đề ảnh hưởng đến tránh thai trong thời kì hậu sản
o Tăng đông (nguy cơ VTE chỉ trở về căn bản sau 42 ngày, và đặc biệt cao
trong 21 ngày)
o Nhiễm trùng
o Nuôi con bằng sữa mẹ
o Phóng noãn không ổn định
Các biện pháp tránh thai
o Estrogen – progestogen ngoại sinh phối hợp (CHC – combined hormonal
contraceptive)
Nguy cơ thuyên tắc mạch do huyết khối (xếp vào loại 4): ngang nhau
giữa những thai phụ nuôi con bằng sữa mẹ và không nuôi con bằng
sữa mẹ không khuyến cáo dùng CHC trong thời gian hậu sản
Ảnh hưởng ngắn hay dài hạn trên trẻ sơ sinh được bú sữa mẹ chỉ
dùng CHC cho đến ít nhất 6 tháng sau sinh nếu nuôi con bằng sữa mẹ
o Progestogen đơn thuần (POC – progestogen only contraceptive): tồn tại chứng
cứ cho rằng POC không có nguy cơ trên sức khỏe và sự phát triển của trẻ sơ
sinh ở những thai phụ nuôi con bằng sữa mẹ DMPA (Depot Medroxy
Progesterone) , NET-EN (Nor-Ethisterone enanthate) là một biện pháp tránh
thai khả dụng cho các sản phụ
o Dụng cụ tử cung (IUD – intra uterine device)
Hạn chế nếu có nhiễm trùng hậu sản
Cu-IUD xếp vào loại 1, LNG-IUD xếp vào loại 2 nếu có nuôi con
bằng sữa mẹ (do LNG-IUD phóng thích levonorgestrel là
progesterone tổng hợp có thể ảnh hưởng đến nuôi con bằng sữa mẹ)
Đặt ngay sau sổ nhau ít bị tống xuất hơn nếu đặt trong thời kì hậu sản,
đặc biệt tỉ lệ tống xuất còn thấp hơn nếu đặt trong mổ sanh so với đặt
trong sanh thường
o Cho con bú vô kinh (LAM – lactational amenorrhoea method)
Dựa trên khả năng ức chế hoạt động buồng trứng khi con bú
3 tiêu chuẩn của WHO buộc phải thỏa để đảm bảo hiệu quả của pp này
Vô kinh
Nuôi con bằng sữa mẹ hoàn toàn
<6 tháng
o Tránh thai theo ngày phóng noãn phỏng đoán (FAB – fertility awareness
based methods) gồm
Dấu hiệu hiện diện của nang noãn trưởng thành (SYM – symptoms
based method)
Phương pháp theo lịch ( CAL – calendar based method)
Tỉ lệ thất bại cao, không khuyến cáo
