MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered into in Mahilum, Hindang, Leyte, Philippines
by and between

The Barangay Council of Mahilum Elementary School, Mahilum, Hindang, Leyte

represented by its Barangay Captain, HON. CRESENCIA M. UDTOHAN. hereinafter referred to as
“The First Party”
- and –
Mahilum Elementary School, represented by its School Head MR. MARVEN F.
FEBRIA, hereinafter referred to as the “Party of the Second Part”;

WHEREAS, The First Party has committed to provide assistance in the implementation of
the Learning Modality to the Second Party;

WHEREAS, Mahilum Elementary School is still implementing Modular Distance

Learning-Printed this school year 2021-2022 where learners accomplished learning tasks at home
with the assistance of home learning facilitators;

WHEREAS, in this learning modality learners have to follow the hours for learning
prescribed per subject per day, and that they should refrain from gallivanting during learning hours;

WHEREAS, the school has to find ways and means to prevent learners from doing so by
regulating/controlling the learners who roam around during learning hours and bring them back to
their respective home and;

WHEREAS, the first party voluntarily commit to provide manpower to do the task;
NOW, therefore, in consideration of the foregoing premises, The Mahilum Barangay
Council and Mahilum Elementary School, hereby agree on and stipulate the following:

A. The First Party shall give support in the form of manpower to the Second Party for the
effective implementation of Printed Modular Distance Learning.

B. The First Party shall report to the school the learners who roam around the barangay during
learning hours for follow-up and monitoring.

IN WITNESS WHEREOF, the parties hereto have signed these presents on this 11 th day of
October, 2021 in Mahilum Hindang, Leyte, Philippines.

Mahilum Elementary School, represented by its principal MR. hereinafter referred to

MARVEN F. FBRIA as the “Party of the Second Part”;

WHEREAS, The Second Party attended one of the council’s sessions and presented the
school’s physical state and seek assistance for the implementation of Printed Distance Modular
Learning, programs, projects and activities;

WHEREAS, the First Party committed to provide assistance in any way they can
(Financial material a moral support;
NOW, therefore, in consideration of the foregoing premises, The Mahilum Barangay
Council and Mahilum Elementary School, hereby agree on and stipulate the following:

A. The First Party shall give support in the form of feeding program to kindergarten
learners as reflected in the annual budget;

B. The First Party shall give their support during Brigada Eskwela, school programs and
other school related activities

C. The Second Party shall also give any kind of support to the barangay especially during
fiesta celebration.

IN WITNESS WHEREOF, the parties hereto have signed these presents on this 22 nd day of
August, 2022 in Mahilum Hindang, Leyte, Philippines.

MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered into in Mahilum, Hindang, Leyte, Philippines
by and between

Mahilum ES Alumni Ass’n, Mahilum, Hindang, Leyte represented by its President,

James Larry M. Udtohan . hereinafter referred to as “The First Party”
- and –

Mahilum Elementary School, represented by its School Head, Marven F. Febria

hereinafter referred to as the “Party of the Second Part”;

WHEREAS, The First and Second Parties had a meeting and discuss about the intention of the
first party to give assistance to the implementation of its programs, projects and activities;

WHEREAS, The First Party as an initial assistance committed to finance the construction of
the school MRF;

NOW, therefore, in consideration of the foregoing premises, Mahilum ES Alumni Ass’n.

and Mahilum Elementary School. hereby agree on and stipulate the following:
A. The First Party shall donate the amount generated from the Alumni Homecoming Night if there’s
any and be used in the implementation of school project;

B. The First Party shall give their support during Brigada Eskwela, school programs and other
school related activities

C. The Second Party shall use the money according to the project proposal approved by the
organization.

IN WITNESS WHEREOF, the parties hereto have signed these presents on this 5th day of
August, 2022 in Mahilum, Hindang, Leyte, Philippines.

- and –

Mahilum Elementary School, represented by its School Head, Marven F. Febria

hereinafter referred to as the “Party of the Second Part”;

WHEREAS, The First and Second Parties had a meeting and discuss about the intention of the
first party to give assistance to the implementation of its programs, projects and activities;

WHEREAS, The First Party as an initial assistance committed to finance the construction of
the school School Teachers’ Cottage;

NOW, therefore, in consideration of the foregoing premises, Mahilum ES GPTA. and

Mahilum Elementary School. hereby agree on and stipulate the following:
A. The First Party shall donate the amount generated from the GPTA Fund if there’s any and be
used in the implementation of school project;

B. The First Party shall give their support during Brigada Eskwela, school programs and other
school related activities

A. The Second Party shall use the money according to the project proposal approved by the
organization.

August, 2022 in Mahilum, Hindang, Leyte, Philippines.
