• Vi khuẩn có trong mảng bám răng là tác nhân chính gây ra bệnh nha chu (Sanders, 1999
). Điều trị chỉnh nha bằng khí cụ cố định được biết là làm tăng thể tích mảng bám răng.
Tuy nhiên, khí cụ chỉnh nha cố định gây ra sự thay đổi loại vi khuẩn (
Petti và cộng sự, 1997 ). Do đó, điều trị chỉnh nha cố định có thể dẫn đến viêm lợi cục
bộ, hiếm khi tiến triển thành viêm nha chu ( Van Gastel và cộng sự, 2007 ).
• Tác dụng nha chu của khí cụ gắn band có thể khác với tác dụng của khí cụ gắn bằng dán
dính, trong đó gắn band có liên quan đến tình trạng viêm tăng lên và mất bám dính khi
so sánh với phương pháp dán (Boyd RL, 1982)
Các khí cụ tháo lắp có thể đè ép trực tiếp lên lợi đặc biệt là vùng lợi sát có răng. Các tay
móc bám qua mặt nhai làm cho các răng bị tách ra và gây nhồi nhét thức ăn dẫn đến tiêu
xương.
• Yếu tố quyết định tình trạng nha chu trong quá trình chỉnh nha chính là mức độ vệ sinh
răng miệng. Vì vậy, hướng dẫn vệ sinh răng miệng nên được đưa ra trước khi bắt đầu
điều trị chỉnh nha và được củng cố trong mỗi lần khám. Vệ sinh răng miệng trong quá
trình điều trị chỉnh nha là chìa khóa để duy trì nha chu khỏe mạnh (
Alstad và Zachrisson, 1979 ).
• Chống chỉ định điều trị chỉnh nha cho những bệnh nhân mắc bệnh nha chu đang hoạt
động vì nguy cơ bị tổn thương nha chu tăng lên rõ rệt (
Zachrisson và Alnaes, 1973; Cardaropoli và Gaveglio, 2007 ). Cần phải đánh giá đầy đủ
tình trạng nha chu, đặc biệt ở bệnh nhân người lớn và cần kiểm soát tình trạng nha chu
trước khi bắt đầu điều trị chỉnh nha.
• Việc kiểm tra cẩn thận mức độ bám dính của lợi trước khi điều trị chỉnh nha toàn diện là
cần thiết. Mức độ bám dính của lợi được đo từ bờ lợi tự do đến biểu mô nối niêm mạc -
lợi trừ đi độ sâu của rãnh lợi. Việc di chuyển răng theo hướng môi-lưỡi có thể được thực
hiện trong phạm vi bao bọc của nha chu mà không gây ảnh hưởng có hại đến mức độ lợi
bám dính ( Wennstorm, 1990 ). Nếu lợi bám dính không đủ mức trước khi điều trị chỉnh
nha, nên thực hiện tư vấn nha chu, đặc biệt nếu dự đoán răng sẽ di chuyển về phía môi (
Wennstorm, 1990, 1996 ).
