8-1 Health and environment department works to
prevent the occurrence of a serious accident and
exposure of workers to injuries. Therefore, periodic
inspection is carried out on the various workplaces and
24 hours a day during work, writing reports on this in
the various workplaces, ensuring the implementation of
the issued instructions and setting appropriate control.
8-2 Near misses that do not result in damage or
loss, and it is important not to neglect them
when reviewing safety records, because they
may indicate risks for which no control
measures are taken. It can lead to serious
accidents, but when such accidents are
reported and investigated, it gives an
opportunity for the facility to take changes to
improve safety before the damage occurs. It

establishments that have a reporting system
and reports on errors or near misses have fewer
accidents and less seriousness. Where if we
look at the Fig. 1 (Bird’s Accident Ratio
Triangle (, we find that facilities that have near
misses are more likely to experience serious
accidents.
8-3 In the event of an injury to one of the
workers in the facility, the Department of
Safety and Environment or the clinic shall be
informed and send a prepared form with
detailed description of injury (Work Injury
Report EHS/O/XXX). In case of road
accidents, a police report must be issued
instead of this form immediately and before
transferring the injured or at the government
hospital to which he is transferred
8-4 In the event of a serious accident, safety
and environment manager or his
representative shall go to inspect the accident
and inform the facility’s occupational safety
and health committee for inspection with the
functional organization health and safety
members and invite the committee to convene
and conduct a primary investigation of the
accident to reach the root causes that led to its
occurrence with the (Risk Assessment) and the
development of control measures to prevent
the recurrence of the accident again according
to the form (EHS/O/XX) and submit a report
to the Occupational Safety and Health Office
in charge of the ministry of Manpower of the
serious accident within 24 hours of the
occurrence of the serious accident inside the
facility.

to the health and safety department manager to
take the necessary action, explaining in it the
complete data of the injured worker, the date,
time and place of the injury As well as the
injured organ , how the injury occurred, what
was taken towards the injured, the apparent
causes that led to the injury, the analysis of the
root causes of the injury, and the
recommendations made to prevent the
recurrence of the injury again. A risk
assessment is also re-assessed after that, after
putting control and control solutions in place,
and a copy of the report is saved in safety and
environment department records .
